Evaluation of Histological and Functional Differences of Interpositional Carotid Bypass Grafts in an Experimental Rat Model

Özet
Amaç: Serebral revaskülarizasyon ameliyatları kafa içi yapılara olan kan akımını arttırmak yada kan akım yönünü değiştirmek için uygulanılmaktadır. İdeal greft tipleri halen tartışmakonusudur nitekim farklı akım miktarlarına ve açık kalma oranlarına sahiptir ve her zamanistenen boyutlarda sağlanamamaktadır. Bu çalışma ile farklı greft tiplerinin uygulanılması ileortaya çıkan histolojik değişiklikler ve patofizyolojik mekanizmaların bir deneysel karotidbypass modelinde araştırılması amaçlanmıştır.Yöntem: Bu çalışmaya 28 adet Wistar sıçan dahil edilmiştir. Sıçanlar greft tiplerine göre 4gruba ayrılmıştır. Bu gruplar primer uç-uça anastamoz, arterial, venöz ve politetrafloroetilengreftleridir. Sıçanlar 3 hafta boyunca gözlenmiş ve daha sonra sakrifiye edilerek anjiografikve histolojik olarak değerlendirilmiştir.Bulgular: En yüksek açıklık oranları arterial greftler ile edinilmiştir (%85.7), bunu venöz(%42.8) ve PTFE greftleri (%14.2) takip etmiştir. Greft tıkanması ile ilişkili en önemlimekanizma fibrintrombüs oluşumu bulunmuştur ancak tunika medya tabakasındaki hasar veenflamasyon düzeyleri de süreçte rol almaktadır.Sonuç: Bu çalışmada bypass cerrahisinde gerek greftin manipulasyonu gerekse açık kalmaoranları ile arterial greftlerin en uygun donör olduğu görülmüştür. PTFE greftleri bu halleri ileek bir kaplama ya da ilaç tedavisi olmaksızın bypass cerrahisinde alternatif bir greft olmaktanuzaktır.
Abstract
Background: Cerebral revascularization procedures are performed in order to increase ordivert the origin of cerebral blood flow (CBF) to intracranial structures in treatment ofcomplex cerebral aneurysm or skull base tumors. The ideal graft type is still a topic of conflictas different graft types possess different rates of flow (high vs. low) and patency, moreover,availability. The following study was designed to investigate histological changes andpathophysiological mechanisms underlying the patency rates in the different graft types(biological vs. artificial) in an experimental model of Carotid bypass.Methods: Twenty-eight Wistar rats were divided into 4 groups based on graft types; primaryend-to-end anastomoses, arterial, venous and polytetrafluoroethylene (PTFE) interpositiongrafts. The rats were followed for 3 weeks and the subjects were sacrificed for histologicaland angiographic examinations.Results: The highest rates of patency was observed in arterial grafts (85.7%), followed byvenous (42.8%) and PTFE (14.2%). The most important mechanism underlying the graftfailure was the fibrin and thrombus formation, whilst, the degree of damage in tunica mediaand inflammation were also correlated.Conclusions: This study showed that the arterial grafts are the most convenient graft type inrespect to graft manipulation and patency rates. PTFE grafts without additional coating ormedications are not alternative grafts for bypass surgeries yet.
