Subdural Empyema in the Adjacent Cortex of an Arteriovenous Malformation

Özet

Arteriovenöz malformasyon ile subdural ampiyemin birlikte görülmesi çok nadirdir.Olgu: Şiddetli başağrısı şikayeti ile başvuran 40 yaşında erkek olgunun bilgisayarlı tomografiincelemesinde sağ temporal orta hat şifti yapan temporal intraserebral hematom saptandı.Cerrahi sırasında organize kapsül ve subdural ampiyem, püy ve trabeküller görüldü. Buenfeksiyon dokusu rezeke edildikten sonra intraserebral hematoma ulaşıldı ve boşaltıldı,AVM nidusu alındı. İyileşme sorunsuz oldu. Bu olgu ışığında ampiyem ve AVM'nin aynıhemisferde oluşumu ile ilgili muhtemel mekanizmalar ve çalma fenomenine bağlıhemodinamik, iskemik süreçler incelendi.Sonuç: Vasküler endotelyal büyüme faktörü ve basic fibroblastic büyüme faktörü gibimoleküler biyolojik faktörlerin anjiojenezi uyarmadaki proliferative ve enflamatuar rollerininetkisi tartışıldı.

Abstract

Background: The coexistence of arteriovenous malformation and subdural empyema isextremely rare.Case Report: A 40-year-old man was admitted with a severe headache. His cranialcomputerized tomography findings showed a right-sided temporal intracerebral hematomawith a midline shift. During surgery, an organized capsule and subdural empyema, whichcontained pus and trabeculae, were seen as the dura was elevated. After resection of theinfectious tissue, the intracerebral hematoma was reached and evacuated, and the nidus of thearteriovenous malformation (AVM) was resected. The recovery was uneventful. Herein, wediscuss the formation mechanism of empyema, together with the AVM in the samehemisphere, and the possible role of the hemodynamic and ischemic processes associated withthe steal phenomenon. Additionally, we present the proliferative and inflammatory roles ofthe molecular biological factors; vascular endothelial growth factor (VEGF) and basicfibroblastic growth factor (bFGF) promoting angiogenesis in AVM.Conclusion: Additionally, we present the proliferative and inflammatory roles of themolecular biological factors; vascular endothelial growth factor and basic fibroblastic growthfactor promoting angiogenesis in AVM.
