Effects of Statin Treatment on Early Outcomes and Restenosis after Carotid Stenting

Özet
Giriş: Karotis stentlemesi yapılan hastalarda statinin yararları iyi tanımlanmamıştır. Buçalışmanın amacı, statinin karotis stentleme sırasındaki serebrovasküler komplikasyonlara vesonrası kısa dönemdeki restenoz oranlarına etkisini belirlemektir.Yöntem: Aralık 2010 ve Aralık 2012 tarihleri arasında karotis stentleme yapılan ardışık 59hastanın kayıtları retrospektif olarak incelenmiştir. Giriş Ulusal Sağlık Enstitüsü Strok Skalası(NIHSS) ve karotis stentleme öncesi ve sonrası modifiye Rankin skalası (mRS) skoru, işlemsırasındaki serebrovasküler komplikasyonları ve restenoz oranları kaydedilmiştir.Sonuç: Karotis stentleme yapılan toplam 59 hasta çalışmaya alınmıştır (%74.6 erkek,ortalama yaş 67.3 , %84.7 semptomatik); 27 (%46.6) hasta statin tedavisi kullanmaktadır.Karotis stentleme öncesi statin tedavisi almakta olan 3 hastada minör inme gelişmiştir(p=0.09). Karotis stentleme sonrasında 24 saat içinde karotis stentleme öncesi statin tedavisialmayan bir hasta majör inme olmuştur (p=0.54). Yirmi iki hastanın takip muayeneleri vardırve karotis stentleme öncesi statin tedavisi almayan sadece bir hastada anlamlı restenozgelişmiştir (p=0.219).Tartışma: Bu çalışmada, karotis stentleme öncesi statin tedavisi ile işlem sırasındakiserebrovasküler komplikasyonlar ve restenoz oranları arasında anlamlı ilişki saptanmamıştır.Statinin, işlem sırasındaki serebrovasküler komplikasyonlara ve karotis stentleme sonrasırestenoza etkisini anlamak için daha fazla hasta içeren çalışmalara ihtiyaç vardır.
Abstract
Introduction: The benefits of statin treatment in patients undergoing carotid angioplasty withstenting (CAS) have not been well established. This study aimed to determine the short-termeffect of statins on periprocedural cerebrovascular complications and on the restenosis ratefollowing CAS.Methods: The medical records of 59 consecutive patients who underwent CAS fromDecember 2010 to December 2012 were retrospectively evaluated. The National Institutes ofHealth Stroke Scale (NIHSS) and modified Rankin Scale (mRS) scores were recorded beforeand after the CAS, and the periprocedural cerebrovascular complications and restenosis rateswere also recorded.Results: Of the 59 CAS patients included in the study, 74.6% were male (mean age, 67.3years; 84.7% symptomatic), and 27 (46.6%) patients were actually undergoing statintreatment. Minor stroke developed in three patients undergoing statin treatment prior to CAS(p=0.09). Major stroke developed within 24 h after CAS in one patient who did not undergostatin treatment prior to CAS (p=0.54). Twenty-two patients had follow-up examinations, andonly one patient who did not undergo statin treatment developed remarkable restenosis(p=0.219).Discussion: No significant associations were observed between the patients who underwentstatin treatment prior to CAS and experiencing periprocedural cerebrovascular complicationsor restenosis. Future studies with larger patient groups are needed confirm the effects ofstatins on periprocedural cerebrovascular complications and restenosis after carotid stenting.
