Özet
Holoprosensefali nadir görülen bir konjenital anomali olup, prozensefalonun gelişiminde bozukluk ile karakterizedir. Genellikle tek ventrikül, korpus kallozum yokluğu, major prozensefalon disgenezisi ve yüz anormallikleri olaya eşlik eder. Holoprozensefali nadir görülmekle beraber diabetik gebelerde artmış bir risk söz konusudur. Öte yandan, oral hipoglisemik ajanların da kulak deformitelerine yol açabileceği bildirilmiştir. Kliniğimize 32. gebelik haftasında refere edilen ve tüm gebeliği boyunca gliklazid kullanan olguda, ultrasonografik olarak ve patolojik incelenmesinde "semilobar holoprozensefali" ve "mikrotia" saptandı. Bu olgu ile birlikte diabetik gebelerde konjenital anomali riskini azaltmak amaçlı konsepsiyon öncesi danışma ve organogenezis sırasında oral hipoglisemik ajanlardan kaçınılması üzerinde tartışıldı.
Abstract
Holoprosencephaly is a rare entity that results from forebrain development disordered and characterized by the presence of a single ventricle, absence of corpus callosum, major farebrain dysgenesis and facial abnormalities. Although it is rare, the risk of holoprosencephaly is increased several folds in pregnant women with diabetes mellitus. Furthermore, it is suggested that oral hypoglycemic agents may cause ear deformities. A pregnant woman was referred to our center at 32 weeks of gestation. She was using gliclazide up to that gestational week. Semilobar holoprosencephaly and microtia were detected ultrasonographically. Regarding to this case report, the importance of preconceptional counselling for reducing fetal congenital malformations in diabetic pregnant women and importance of avoiding the use of oral hypoglycemic agents during organogenesis were discussed
