Özet
AMAÇ: Intrasitoplazmik sperm enjeksiyonu (ICSI) ile elde edilen tekil gebeliklerde ikinci trimester maternal serum total human koryonik gonadotropin (HCG), alfa- feto protein (AFP) and serbest östriol (sÖ3) konsantrasyonlarının değerlendirilmesi. MATERYAL ve METOD: ICSI ile tekil gebelik elde edilen 24 olguda 16-20. gebelik haftaları arasında serum AFP, sÖ3 ve HCG konsantrasyonları ölçülerek MoM değerleri hesaplandı. Değerler aynı zaman diliminde spontan gebe kalarak hastanemiz laboratuvarında üçlü tarama testi yaptıran 82 tekil gebenin sonuçları ile karşılaştırıldı. BULGULAR: Çalışma ve kontrol grupları arasında anne yaşı ve kilosu, gebelik haftası, sigara alışkanlığı açısından fark yokken kontrol grubunda parite daha yüksekti. ICSI gebeliklerinde HCG, AFP ve sÖ3 için MoM değerleri sırasıyla 1.42±0.04, 0.99±0.04 ve 1.00±0.01 olarak saptandı. AFP ve sÖ3 MoM değerleri kontrol grubu ile benzer iken, total HCG MoM değeri kontrol grubundan anlamlı olarak daha yüksekti. Hiçbir olguda tarama pozitifliği saptanmadı. Kromozomal anomalili bebek doğumu, fetal ve neonatal ölüm tespit edilmedi. SONUÇ: ICSI ile elde edilen tekil gebeliklerde artmış HCG seviyeleri fazla sayıda embriyo transferi ve corpus luteum oluşumuna, ICSI uygulamasına bağlı maternal hormonal çevreye veya diğer faktörlere bağlı olabilir. HCG düzeyindeki bu artış Down's Sendromu açısından yanlış pozitif tarama sonuçlarına neden olabilir. Yardımcı üreme teknikleri ile elde edilen gebeliklerde en ideal Down's Sendromu tarama testi için ileri çalışmalara ihtiyaç vardır.
Abstract
OBJECTIVE: To evaluate second trimester maternal serum total human chorionic gonadotrophin (HCG), alpha-feto protein (AFP) and unconjugated estriol (uE3) concentration in singleton pregnancies conceived with intracytpolasmic sperm enjection (ICSI). STUDY DESIGN: Second trimester (16-20 weeks) maternal serum levels of AFP, uE3 and HCG were measured and expressed as multiples of median (MoM) values in 24 patients conceiving singleton pregnancies by ICSI. The results were compared with those of 82 naturally occuring singleton pregnancies who had serum screening at 16-20 weeks at the same laboratory over the same period of time. RESULT(s): There were no significant differences between the study and control groups in maternal weight, age, gestational week and habitus of cigarette smoking. For the ICSI patients, parity was higher than that of the controls. The median levels of the triple markers were 1.42±0.04 MoM for HCG, 0.99±0.04 MoM for AFP and 1.00±0.01 MoM for uE3 in the ICSI pregnancies. The values of AFP and uE3 were not significantly different from that in controls, but total HCG was significantly increased in ICSI pregnancies. No women were found to be screen positive. In neither group were chromosomal abnormalities detected and no fetal or neonatal deaths were recorded. CONCLUSION(s): The biologic basis of the elevated HCG levels may be related to the greater number of transferred embryos and corpora lutea, the maternal hormonal environment of the ICSI process or other factors. The increased HCG levels may contribute to the higher Down's Syndrome screen positive rate in this ICSI singleton population. Further studies are needed to determine the best screening method for Down's Syndrome in the pregnancies obtained by assisted reproductive techniques.
