Non immun hidrops fetalis etyolojisinde yer alan kromozom anomalilerinin spektrumu

Özet

AMAÇ: Fetusta hidropsa yol açan kromozom anomalilerinin non immun hidrops fetalis etyolojisi içerisindeki oranını, spektrumunu, bunlara eşlik eden yapısal anomalileri ve bu fetusların perinatal sonuçlarını belirlemek. MATERYAL ve METOD: Kliniğimiz prenatal tanı ünitesinde ultrason ile non immun hidrops fetalis tanısı alan ve kromozom anomalisi saptanan 43 olgunun verileri gözden geçirildi. Kromozom anomalisi spektrumu, beraberinde görülen yapısal anomaliler ve prognoz verileri dokümante edildi. BULGULAR: Non immun hidrops fetalis saptandıktan sonra karyotip analizi yapılabilen 311 olgunun 43'ünde (% 9) kromozom anomalisi saptandı. On yedi olguda (% 39.5) Turner sendromu, 14 olguda (% 32.5) trizomi 21, 4 olguda (% 9) translokasyon, ikişer olguda (% 4.5) delesyon , trizomi 18, mozaizm ve birer olguda (% 2.2) trizomi 16, trizomi 13 bulundu. Kromozom anomalisi saptanan fetuslardan 35'inde (% 81) aynı zamanda yapısal anomali saptandı. Kromozom anomalili olguların 30'unda gebelik sonlandırıldı, 9 olgu in utero ölü fetus ve 4 olgu neonatal ölümle sonuçlandı ve sağkalım olmadı. SONUÇ: Non immun hidrops fetalis ultrasonografi ile tanısı kolay konan ancak heterojen etyolojisi olması nedeniyle altta yatan nedenin ortaya konulmasının güç olduğu bir durumdur. Kromozom anomalileri etyolojide önemli yer tutmaktadır ve varlığında prognoz olumsuzdur. Non immun hidrops fetalis tanısı alan olgularda sebep araştırılırken yapılması gerekli en önemli incelemelerden biri fetal karyotip analizidir.

Abstract

OBJECTIVE: To define the ratio and spectrum of chromosomal abnormalities causing hydrops and non immune hydrops fetalis and to search the coexisting structural anomalies and their perinatal outcomes. STUDY DESIGN: Records of 43 non immune hydrops fetalis cases with chromosomal abnormalities were reviewed. The spectrum of chromosomal abnormalities, coexisting structural anomalies and the prognosis of the fetuses were analyzed. RESULT(s): Among chromosomally-analyzed 311 non hydrops fetalis cases, 43 fetuses were found to be abnormal: Turner syndrome (n=17; 39.5 %), trisomy 21 (n=14; 32.5 %), translocation (n=4; 9 %), deletion (n=2; 4.5 %), trisomy 18 (n=2; 4.5 %), mosaicism (n=2; 4.5 %), trisomy 16 (n=1; 2.2 %), and trisomy 13 (n=1; 2.2 %). Coexisting structural defects were found in 81 % (n=35) of the fetuses. Among 43 cases with chromosomal abnormalities, 30 cases were terminated, 9 cases died in utero and 4 cases died in the neonatal period. CONCLUSION(s): The ultrasonographic diagnosis of non immune hydrops fetalis is easy but uncovering the underlying etiology is not possible in all cases. Chromosomal abnormalities take an important place in the etiology and serve as a poor prognostic factor. Therefore, it is necessary to perform fetal karyotyping in the process of diagnostic work-up for non immune hydrops fetalis cases.
