Özet

AMAÇ: Endometriyal karsinom kadın genital sisteminin tüm dünyada görülen en yaygın invaziv neoplazmıdır. Sitolojinin yaygın tarama yöntemi olarak kullanılma kolaylığı ve yüksek oranlara varan duyarlılığı endometriyal sitolojiyi tarama ve erken tanı yöntemi olarak gündeme getirmiştir. Biz pek çok çalışma sonucu en ideal endometriyal sitolojik örnekleme yöntemi olarak kabul gören TAO fırçasının endometriumdaki tanı değerini araştırdık. MATERYAL ve METOD: Hastanemiz Kadın Hastalıkları ve Doğum Kliniğinde 1999-2000 yıllarında ameliyat edilen 75 histerektomi piyesinde çalışıldı. Postoperatif taze histerektomi piyeslerinden hazırlanan sitolojik preparatlar TAO'nun belirlediği kriterlere göre değerlendirilip, sonrasında hazırlanan histolojik kesitler ile karşılaştırıldı. BULGULAR: Sitolojik preparatlarda 75 olgunun 62 tanesi benign endometriyum, 2 tanesi basit hiperplazi, 1 tanesi atipik hiperplazi, 8 tanesi ise endometriyal karsinom tanısı aldı. İki olgu sitolojik değerlendirme için yeterli hücre içermiyordu. Histolojik kesitler ile karşılaştırıldığında malign ve benign ayrımının % 100 oranında yapıla-bildiği ancak tanısal düzeyde bu oranın % 79.03 olduğu görüldü. SONUÇ: Sonuçlarımız benign ve malign lezyonların ayrımında bu çalışmanın başarılı olduğunu ortaya koymaktadır. Bu yöntem klinik olarak iyi tolere edilebilen etkili bir yöntemdir. Deneyimli klinisyen ve patolog işbirliği ile yaygın kullanılabildiğinde başarılı sonuçlar elde edilebileceğine inanıyoruz.

Abstract

OBJECTIVE: Endometrial carcinoma is the most frequent invasive neoplasm of the female genital tract. The widespread use of cytology as a screening method and its high sensivity has resulted in using the endometrial cytology as a screening tool for early diagnosis. In this study, we have investigated the diagnostic value of TAO brush for endometrial lesions, which has been accepted as the ideal cytologic sampling method in many studies. STUDY DESIGN: Seventy five hysterectomy specimens belonging to patients, operated between years 1999-2000 were used for the study. Endometrial cytologic samples from fresh hysterectomy specimens were evaluated according to TAO criteria and the results were compared with the histological sections. RESULT(s): Among 75 cytologic preparations, 62 cases were diagnosed as benign, two as simple hyperplasia, one as atypical hyperplasia and eight cases as endometrial carcinoma. Two samples showed inadequate cellularity. When histological and cytological specimens were compared, benign lesions could be differentiated from malignant ones in all of the cases. However, the detailed diagnosis of nonmalignant cases could be done in only 79.03 % of the nonmalignant cases. CONCLUSION(s): Our results have shown that this met-hod is effective in differentiating malignant and benign cases and is a clinically well tolerated method and can be successfully used with the collaboration of experienced clinicians and pathologists.
