Özet

AMAÇ: Vitamin B12 ve folik asit, nükleik asit ve DNA sentezinde birbirine bağımlı rolleri olan iki önemli vitamindir. Güncel çalışmalarda, nöral tüp defekti, spontan abortus, intrauterin gelişme geriliği, düşük doğum ağırlığı, preeklampsi, intrauterin ölüm, plasenta dekolmanı gibi obstetrik patolojiler ile olan ilişkisinin üzerinde durulmaktadır. Bu çalışmanın amacı Şanlıurfa ilinde yaşayan bir grup gebe kadında serum düzeylerine göre folik asit ve vitamin B12 eksikliği prevalansını tanımlamak, ileri çalışmalara temel oluşturarak, obstetrik ve infantil dönemdeki oluşabilecek patolojileri önlemektir. MATERYAL ve METOD: Çalışmaya, rutin antenatal izlem için gebe polikliniğimize başvuran 275 sağlıklı gebe dahil edildi. Olguların hiçbiri vitamin türevi ilaç kullanmıyordu. Serum B12 ve folik asit düzeyleri elektrokemilüminessey yöntemi ile ticari kit kullanılarak, otomatik kemilüminessey analizöründe ölçüldü. Çalışmanın istatistik analizi; student's-t testi ve pearson korelasyon analizi ile, belirtici istatistik değerlendirilmesi; SPSS 11.02 for Windows programı kullanılarak yapıldı. p

Abstract

OBJECTIVE: Vitamin B12 and folic acid are the two important vitamins that have interdependent roles in mucleic acid synthesis. There are current studies on the associations of obstetric pathologies (such as neural tube defects, spontaneous abortion, intrauterine growth retardation, low birth weight, preeclampsia, intrauterine death, and abruptio placenta) with vitamin B12 deficiency. The aims of this study were to determine folic acid and vitamin B12 deficiency in pregnant women in Şanlıurfa and to obtain basic information for further studies on prevention of obstetric and infantile pathologies. STUDY DESIGN: Blood samples of 275 healthy pregnant women were studied during their first prenatal visit. None of them were using oral vitamin formulae. Serum B12 and folic acid levels were measured by using electrochemiluminescence method with commercial kits and with an automatic chemiluminassay analyzer. Student's-t and Pearson correlation tests were used for statistical analysis. RESULT(s): It was found that 54.7 % (123 cases) of the pregnant women had low serum levels of vitamin B12 and 8.7 % (24 cases) of them had low serum folic acid levels. Peripheral blood smear of 9 (40.9 %) cases with severe vitamin B12 deficiency (n=22.8 %) were consistent with megaloblastic anemia. CONCLUSION(s): The incidence of patients who had low B12 levels was high. Peripheral blood smear is a safe and effective method for diagnosing severe vitamin B12 deficiency. In general, adding small amounts of vitamin B12, such that currently found in most over-the-counter multivitamin drugs, would be a good choice. Additionally, consumption of meat products and improving socioeconomic status may be of help in preventing vitamin B12 deficiency in pregnant women.
