Endometriyal patolojilerin belirlenmesinde histeroskopinin yeri: Tanısal önemi ve diğer tanısal testlerle karşılaştırılmasında bir üniversite kliniğinin deneyimi

Özet
AMAÇ: Yapılan histeroskopi işlemleri retrospektif olarak gözden geçirilerek, anormal uterin kanama tanısıyla histeroskopi uygulanan olgularda tanısal testlerin (Trans-vaginal ultrasonografi/TVUSG, Salin İnfüzyon sonografi/SİS, Histeroskopi) kliniğimiz için doğruluk oranları ve verimlilikleri ortaya koyulmaya çalışıldı.MATERYAL ve METOD: Çalışma retrospektif olarak İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalın'da 1.1.1997 ve 31.12.2002 yılları arasında hasta dosyalarının incelenmesi ile oluşturuldu. Tanısal ya da operatif histeroskopi uygulanmış olan toplam 385 olgunun dosyası gözden geçirildi. Olguların isimleri, yaşları, menopozal durumları, obstetrik anamnezleri, özgeçmişleri, yakınmaları, transvaginal ultrasonografi, salin infüzyon sonohisterog-rafi ve histeroskopi özellikleri ve bulguları, alınmış biyopsi ya da rezeksiyon materyallerinin histopatolojik değerlendirme sonuçları kaydedildi. SİS ve histeroskopinin duyarlılık, özgüllük, pozitif belirleyici değer ve negatif belirleyici değerleri, işlemler sırasında saptanan bulgular ve histopatoloji sonuçları karşılaştırılarak hesaplandı. Her bir patoloji için tanısal doğruluk oranları ayrı ayrı hesaplandı.BULGULAR: Olguların yaş ortalaması 38±10'dur. Ol-guların 321'i premenopozal, 64'ü postmenopozal dönemdedir. Histeroskopi endikasyonları arasında, ilk üç sırada endometriyal polip (215 olgu, % 55), infertilite (89 olgu, % 23) ve submüköz myom (33 olgu, % 9) yer almaktadır. Histeroskopi sırasında rezeke edilen materyallerin pato-lojik incelemesinde, 168 olguda endometriyal polip (% 43.5) saptanırken, 108 olguda alınan biyopsi sonucu normal (% 28) olarak belirlenmiştir.Endometriyal poliplerin belirlenmesinde histeroskopinin sensitivitesi % 83, spesifisitesi % 63 olarak hesaplanmıştır. Yalancı negatiflik oranı % 16 ve yalancı pozitiflik oranı % 36'dır. Endometriyal poliplerin belirlenmesinde ise, SİS'nin sensitivitesi % 86 ve spesifitesi % 28 olarak belirlenmiştir. Yalancı negatiflik oranı % 12 ve yalancı pozitiflik oranı % 71'dir.Submüköz miyomların belirlenmesinde histeroskopinin sensitivitesi % 80, spesifisitesi % 91 olarak hesaplanmıştır. Yalancı negatiflik oranı % 20 ve yalancı pozitiflik oranı % 9.8'dir. Endometriyal poliplerin belirlenmesinde ise, SİS'nin sensitivitesi % 76 ve spesifitesi % 92 olarak belirlenmiştir. Yalancı negatiflik oranı % 23 ve yalancı pozitiflik oranı % 7'dir.Histeroskopi uygulanan olguların % 97'sinde hiçbir peroperatif ve % 100'ünde hiçbir postoperatif komplikasyon saptanmazken, komplikasyon olan olguların (11 olgu) % 81.8'i premenopozal olgulardır (9 olgu). Dokuz olgunun 4'ünde uterus perforasyonu olmuştur, 3'ünde ise kaviteye girilememiştir. Bir olguda servikal laserasyon oluşurken, bir olgu histeroskopi sırasında eksitus olmuştur. Postmenopozal olgulardan 2'sinde komplikasyon oluşmuştur. Bu olguların birisinde uterus perforasyonu olurken, diğerinde kaviteye girilememiştir.SONUÇ: Tanısal yeterliliğinin yüksekliği ve düşük komplikasyon oranları nedeniyle, histeroskopinin endometriyal patolojilerin tanısı ve tedavisinde, 'altın standard' olma özelliğini uzun süreler koruyacağı kanısını taşımaktayız.
Abstract
OBJECTIVE: The hysteroscopic procedures in our clinic were retrospectively reviewed in order to reveal the diagnostic accuracy and the efficacy of various tests such as transvaginal ultrasonography (TVUSG), saline infusion sonography (SIS), and hysteroscopy in the management of abnormal uterine bleeding and infertility.STUDY DESIGN: The study was completed in Istanbul University Cerrahpasa School of Medicine, Department of Obstetrics and Gynecology, by reviewing the medical records of the patients underwent hysteroscopy between 1.1.1997 and 31.12.2002. The records of 385 patients were reviewed. Their names, ages, menopausal status, obstetric and medical history, complaints, the characteristics of hysterosalpingography, saline infusion sonography and hysteroscopy and the results of the histopathological examinations were noted. The sensitivity, specificity, positive and negative predictive values for saline infusion sonography and hysteroscopy were calculated for each histopathological diagnosis.RESULT(s): The mean age of the patients was 38±10 years. Three hundred and twenty-one patients were premenopausal and 64 were postmenopausal. The major indications were endometrial polyps (215 cases, 55%), infertility (89 cases, 23%) and submucous fibroids (33 cases, 9%). The histopathological examination of the specimens obtained during hysteroscopic procedures revealed endometrial polyps in 168 cases (43.5%) and reported as normal in 108 cases (28%). The sensitivity and specificity of hysteroscopy for detecting endometrial polyps were 83% and 63%, respectively. False negative rate was 16 % and false positive rate was 36%. The sensitivity and specificity of saline infusion sonography for detecting endometrial polyps were 86% and 28%, respectively. False negative rate was 12% and false positive rate was 71%. The sensitivity and specificity of hysteroscopy for detecting submucous fibroids were 80% and 91%, respectively. False negative rate was 20% and false positive rate was 9.8%. The sensitivity and specificity of saline infusion sonography for detecting submucous fibroids were 76% and 92%, respectively. False negative rate was 23% and false positive rate was 7%.No peroperative complication was observed in 97% of the cases. Nine of the complications occurred in the premenopausal whereas two in the postmenopausal patients. Uterine perforation was detected in four cases; intrauterine cavity could not be entered in three of the cases. Cervical laceration in a case and death was noted in another case. In postmenopausal patients, one uterine perforation and one inability to enter the uterine cavity was noted.CONCLUSION(s): Due to its high diagnostic accuracy and lower complication rate, we believe that hysteroscopy will retain its place as the gold-standard procedure for investigating endometrial pathologies.
