Özet

AMAÇ: Semende lökosit analizi, erkek genital infeksiyonları ve infertilitesinin teşhisinde önemlidir. Rutin semen lökosit analizi, direkt mikroskopik gözlem ve peroksidaz testi ile yapılmaktadır. Son zamanlarda flow sitometri de, androloji laboratuvarlarında semen analizi için kullanılmaktadır. Bu çalışmanın amacı, flow sitometrik analizi direkt mikroskopik gözlem ve peroksidaz testi ile karşılaştırmaktır.MATERYAL ve METOD: Semen analizleri 45 erkek hastadan alınan ejakulat örneklerinde yapıldı. Semen lökosit değerlendirmesinde direkt mikroskopi, peroksidaz testi ve flow sitometri yöntemi kullanıldı. Flow sitometrik yöntem referans olarak alınarak polimorfonükleer lökositleri tespit etmede, direkt mikroskopik yöntem ve peroksidaz testinin sensitivite ve spesifitesi değerlendirildi.BULGULAR: Flow sitometri referans test (gold test) olarak alındığında, direkt ışık mikroskopik sayım % 60.6 sensitivite ve % 83.3 spesifite, peroksidaz testi ise % 81.8 sensitivite ve % 91.6 spesifite gösterdi.SONUÇ: Monoklonal antikorlar kullanılarak yapılan flow sitometrik analiz, direkt ışık mikroskopik sayım ve peroksidaz testine göre semendeki lökositleri tam olarak gösterebilen daha objektif bir metoddur.

Abstract

OBJECTIVE: The analysis of leukocytes in human semen could be useful in diagnosing male genital infections and infertility. Routine seminal leukocyte analysis is carried out by direct microscopic examination and peroxidase test. Recently, flow cytometry has been started to use for semen analysis in andrology laboratories. The purpose of this study was to compare flow cytometry to direct microscopic examination and the peroxidase test for seminal leukocyte analysis.STUDY DESIGN: Samples of ejaculated semen from 45 male patients were analyzed. Direct microscopy, peroxidase test and flow cytometric analysis were performed for seminal leukocyte analysis. Sensitivity and specificity of direct microscopic methods and peroxidase test for polymorphonuclear leukocytes were studied considering flow cytometry as the reference technique.RESULT(s): When flow cytometry is taken as reference technique, direct light microscopy had 60.6% sensitivity and 83.3% specificity, and peroxidase test had 81.8% sensitivity and 91.6% specificity in evaluating seminal samples for leukocytes.CONCLUSION(s): We have concluded that flow cytometric analysis using monoclonal antibodies is a better objective method than direct microscopy and peroxidase test, enabling seminal leukocytes to be accurately detected.
