Total tiroidektomili bir hastada, over tümör rezeksiyonu sonrası gelişen hipotiroidizm

Özet
Bu olgu sunumunda, 52 yaşında postmenopozal bir hastada saptanan pelvik kitle-struma ovarii ele alındı. Elli iki yaşında postrnenopozal total tiroidektomi operasyonu geçirmiş olan, karında şişlik yakınması ile jinekolojik onkoloji bölumüne başvuran pelvik muayenede semisolid yapıda kitle saptanan hasta opere edildi. Histopatolojik inceleme sonrasında kitlenin ektopik tiroid dokusu-struma ovarii olduğu görüldü. Preoperatif incelemede hastanın tiroid fonksiyonlarının normal sınırlar içinde olduğu gözlendi (TSH=0.6 ug/dl, T3=1.03 ug/dl, T4=10 ug/dl). Postoperatif 5. günde yapılan kontrolde TSH düzeyinin yükseldiği (54.08 uIU/ml), T3 ve T4 değerlerinin düştüğü (T3=0.3 ng/ml, T4= 1.53 ug/dl) saptandı. Bu bulgular struma ovarii'nin fonksiyonel olduğunu düşündürdü. Operasyondan sonra tiroid replasman tedavisine başlanan hastada tiroid hormon değerleri normale döndü. Tiroidektomi sonrasında tiroid hormon replasmanı almayan tiroid fonksiyonları normal saptanan pelvik kitleli olgularda struma ovarii düşünülmeli.
Abstract
In this report a 52 year-old postmenopausal patient with a pelvic mass which diagnosed as struma ovarii is presented. The patient had previous total tyroidectomy applied to the gynocologic oncology clinic with a complaint of abdominal welling. A pelvic mass diagnosed after pelvic examination and operation was performed. Histopathologic examination of pelvic mass mas diagnosed as struma ovarii. During preoperative evaluation, tyroid function tests of patient were normal (TSH=0.6 ug/dl, T3=0.3 ug/dl, T4=10 ug/dl). During the postoperative evaluation of patient increased TSH leuds (54.08 µIU/ml) and decreased T3 and T4 (T3=0.3 ng/ml, T4=1.53 ug/dl) hormone levels were detected. These laboratory findings suggested us that the pelvic wass which diagnosed as struma ovarii was functional. Tynoid functions of patient returnes normal limits after tyroid replacement therapy during the post-operative follow-up. The diagnose of struma ovarii should be hept in and of the patients with pelvic wass who had normal tyroid function despite of previous tyroidectomy operation
