Özet

Endodermal sinus tümörü (Yolk kesesi tümörü), ekstraembryonel yolk kesesine farklılaşmış germ hücrelerinden kaynaklandığı düşünülen, disgerminomadan sonra ikinci sıklıkta görülen ve malign germ hücreli tümörler içerisinde en malign seyirli olarak bilinen germ hücreli tümördür. Son yıllarda kemoterapi alanındaki gelişmeler ile birlikte bu hastaların yaşam sürelerinde çok önemli aşamalar kaydedilmiş ve erken evre tümörlerde % 95'lere varan 5 yıllık sağkalım süreleri bildirilmeye başlanmıştır. Yapılacak agresif cerrahi girişimi takiben zamanında ve yeterli dozda uygulanan "Bleomycin + Etoposide + Cisplatin" protokolü adjuvan kombine kemoterapi ile ileri evre tümörlerde dahi çok yüz güldürücü sonuçlar elde edilebilmektedir. Endodermal sinüs tümörü tanısı almış hastaların tamamı postoperatif dönemde yapılacak adjuvan "Bleomycin + Etoposide + Cisplatin" protokolu kemoterapi ile tedavi edilmeli ve tedaviye alınan cevabın takibinde Alfa-fetoprotein kullanılmalıdır. Burada 23 yaşında, ileri evre endodermal sinüs tümörü nedeniyle opere edilen ve postoperatif kemoterapi ile tedavi edilen bir hasta 8 yıllık sağkalım süresi nedeniyle sunulmaktadır.

Abstract

Endodermal sinus tumor is the second most common form of malignant germ cell tumors after dysgerminoma which is believed to arise from germ cells that differentiate into the extraembryonal yolk sac and they are highly malignant. During last 3 decades survival rates of these patients have dramatically improved with the advanced chemotherapy regimes and almost 95 % 5 year survival rate has been reported with early stage tumors. Higher survival rates could be obtained with appropriate dose adjuvant combination chemotherapy including "Bleomycin + Etoposide + Cisplatin" after aggressive surgery in advanced staged patients. Currently postoperative chemotherapy with a "Bleomycin + Etoposide + Cisplatin" regimen should be the standard care for all patients with a diagnosis of endodermal sinus tumor and a-fetoprotein should be used for monitoring results of the therapy. We present a 23-year-old woman with advanced stage endodermal sinus tumor who achieved 8 years sustained complete remission with combined adjuvant chemotherapy after surgery.
