Özet

AMAÇ: Çalışmanın amacı, sağlıklı ve preeklamptik gebelerde plasental patolojiyi karşılaştırmaktır.MATERYAL ve METOD: Çalışmaya Ekim 2001 ile Eylül 2002 tarihleri arasında prospektif olarak, preeklampsi-eklampsi tanısı ile yatırılarak takip ve tedavileri yapılan 83 olgu ile sistemik hastalığı olmayan 30 kontrol olgusu dahil edildi. Plasentalar, makroskopik incelemesi yapıldıktan sonra, dokuz histolojik parametre dikkate alınarak ışık mikroskobunda incelendi. İstatistiksel işlemler; student t testi ve x2 (ki-kare) testi ile değerlendirildi.BULGULAR: Sinsityal düğümde artış, sitotrofoblastta proliferasyon, fokal perivillöz fibrin depolanması, villöz stromal fibrozis, uteroplasental damarda fibrinoid nekroz, maturasyon, bazal membran kalınlaşması, bazal membranda mineralizasyon ve villöz stromal ödem çalışma grubunda kontrol grubuna göre istatistiksel olarak anlamlı düzeyde yüksek bulundu. Uteroplasental damarda fibrinoid nekroz, villöz stromal fibrozis, bazal membranda mineralizasyon ve villöz stromal ödem kontrol grubunda hiçbir hastada saptanmadı.SONUÇ: Preeklampsinin patogenezinde plasental patoloji önemli rol oynamaktadır.

Abstract

AİM: The objective was to evaluate placental pathologic features in normal and preeclamptic pregnancies.STUDY DESING: Eighty-three preeclamptic-eclamptic and 30 control pregnant women without systemic disease between October 2001 and September 2002 were prospectively recruited. Placental pathologies were evaluated due to macroscopic findings and nine histological parameters on light microscope. Statistics included student t test and chi-square test.RESULT(s): Increased syncytiotrophoblast knotting, cytotrophoblast proliferation, focal perivillous fibrin deposition, villous stromal fibrosis, fibrinoid necrosis, maturation, basal membrane thickening, mineralization in basal membrane and villous stromal edema were more common in the study group compared to the control group. Fibrinoid necrosis in uteroplacental vessels, villous stromal fibrosis, mineralization in basal membrane and villous stromal edema were not seen in any patient in the control group.CONCLUSION: Placental pathologic features play an important role in the pathophysiology of preeclampsia.
