Özet

AMAÇ: Yaş, parite, vücut kitle indeksi (VKİ) ve menopoz durumunun ürodinamik tanı üzerinde etkisinin araştırılması.MATERYAL ve METOD: Ocak-Haziran 2004 tarihleri arasında idrar kaçırma şikâyeti ile ürojinekoloji kliniğine başvuran 302 hastaya ürodinamik inceleme yapıldı. Olgular herhangi bir sistemik ve nörolojik hastalığı olmayan hastalardan seçildi. Ayrıca, muayene bulgusunda subtotal ve total prolapsus saptanan olgular çalışma dışında bırakıldı. Olgular, yaşa göre 40 yaş ve altı, 40-50 yaş arası, 50 yaş ve üzeri olarak üçe, parite sayılarına göre 3'den az, 3-4 ve 4'ten fazla olarak üçe, VKİ olarak 25 ve altı, 25-30 arası ve 30'dan fazla olarak üçe, menopoz durumuna göre menopoz ve premenopoz grubu olarak ikiye ayrılarak ürodinamik tanılar değerlendirildi. Ürodinamik tanıda, dolum sistometrisi ve üretral basınç profili değerleri kullanıldı. Ürodinamik tanı olarak olgular, anatomik stres inkontinans ve alt grupları (Tip I, II, III ASİ), detrusor instabilitesi, miks tip inkontinans olarak sınıflandırıldı.BULGULAR: Olgular, yaş gruplarına göre gruplandırıldığında ürodinamik tanı olarak gruplar arasında istatistiksel fark saptanmadı (p=0.070). Paritelerine göre gruplandırıldığında yine ürodinamik tanı dağılımı bakımından gruplar arasında istatistiksel fark yoktu (p=0.741). VKİ'ye göre olgular gruplandırıldığında da gruplar arasında istatistiksel fark yoktu (p=0.789). Menopoz durumu dikkate alınarak yapılan gruplandırmada ise, menopoz grubunda Tip III stres inkontinansın ve detrusor instabilitesinin anlamlı olarak daha fazla olduğu görüldü (p=0.021).SONUÇ: Yaş, parite, VKİ ve menopoz durumuna göre yapılan gruplandırmaların ürodinamik tanıya etkileri araştırıldığında, sadece menopoz olması durumunda ürodinamik tanı dağılımında fark oluştu. Menopoz grubunda, menopoz öncesi gruba göre Tip III ASİ ve Detrusor instabilitesi sıklığı daha fazlaydı.

Abstract

AIM: To investigate the effect of age, parity, body mass index (BMI) and menopause on urodynamic diagnosis.STUDY DESIGN: Urodynamic investigation was performed on 302 patients referred to urogynecology unit with urinary incontinance complaint between January and June 2004. Patients had not got any systemic or neurological diseases. Cases with subtotal or total uterine prolaps excluded from the study. The cases were divided in to the three groups according to age as, equal and under the 40 years old, between 40-50 years old and over 50, according to parity they were divided in to three groups as less than 3 parities, 3-4 parities and more than 4 parities. According to BMI the cases were divided in to three groups as, equal and less than 25, between 25-30, equal and more than 30. According to menopause situation, they were investigated as two groups, menopause and premenopause. The urodynamic diagnoses were evaluated according to the case groups. Filling sistometry and urethral pressure profilometry values were used for urodynamic diagnose. The cases were classified as anatomic stres incontinance and its subgroups (type I, II, III ASİ), detrusor instability, mixed type incontinence for urodynamic diagnose.FINDINGS: With the evaluation of the cases according to the age parameter, any istatistical differance was not found between the case groups (p=0.070). When the parity parameter was evaluated for the urodynamic diagnose, the istatistical differance can not be found (p=0.741). According to the BMI groups, no istatistical differance was shown on the urodynamic diagnosis (p=0.789). When the menopause situation was evaluated, we found that in the menopause group, the type III stres incontinence and detrusor instability was meaningly much more present than the premenopause group (p=0.021).CONCLUSION: The effect of age, parity, BMI and menopause on the urodynamic diagnose was investigated. It was found that only the menopause situation made differance on the urodynamic diagnose. Type III ASI and the detrusor instability were more common in the menopause group than the premenopause group.
