Böbrek parankim hastalıklarında real time US ile biyopsi

Özet
Bu çalışmada klinik ve sonografık olarak böbrek parankim hasarı olduğu düşünülen 44 olgu ve solid fokal lezyonu olan 2 olguya real time US kılavuzluğunda tru-cut iğne biyopsisi uygulanmıştır. Alınan biyopsi materyalinin immunoflorosan yöntemle histopatolojik değerlendirmesi sonucu pataloji aydınlatılmış ve olguya yapılacak tıbbi yaklaşım yönlendirilmiştir. Histopatolojik tanı alan 46 olgunun %52'si glomerulonefrit (4 SLE, l mikro-anjiopatik trombositopenik ncfropati, l ilaç toksisitesi), %22 si amiloidoz, %13'ü DM glomerulo sklerozu, %4.8'i IgA nefropatisi, %4.8'i End Stage böbrek hastalığı, %2.2'si kronik pyelonefrite sekonder değişiklik ve solid kitle lezyonu düşünülen 2 olgunun birisinde papiller adenokarsinom, birinde bertin! kolon lıipertrofisi sonuçları gelmiştir. Bu çalışmada böbrek parankim hastalıklarında ve şüpheli kitle lejyonlarında US kılavuzluğunda tru-cut biyopsi tekniğinin tanısal etkinliği ve real time işlemin kör biyopsiye teknik üstünlükleri ortaya konmuştur.
Abstract
in this study, sonographycally guided tru-cut biopsy was performed in 44 cases with renal paranchymal damage and 2 cases with solid fokal lesions. The biopsy materials were histopatologically examined the immunoflourescent method. 52% of the 46 cases were diagnosed to be glomerulonephritis (4 SLE, 1 micro-angiopatic trombocytopenic nephropathy, 1 drug toxicity), 22% had amyloidosis, 13% were diagnosed with DM glomerulosclerosis, 4.8% with IgA nephropathy, 4.8% with endstage renal disease and 2.2% with findings secondary to chronic pyelonephritis. One of the 2 cases with solid renal lesion was papillary adeno-carcinoma and the other was diagnosed to be Bertini column hipertrophy. In this study, the diagnostic effectiviness of sonographycally guided tru-cut biopsy tecnique was evaluated in cases of renal paranchymal desease and suspicions mass lesions and the superiority of this real time tecnique was compared with nonguided biopsy.
