Rektal karsinomların preoperatif endorektal ultrasonografi ile evrelendirilmesi

Özet
Çalışmada İAEH'nde hospitalize olan rektum karsinom tanılı 30 hastaya endorektal US yapılmıştır. Rektum Ca'larda uygun cerrahi yöntemin seçilebilmesi veya inoperabilite kriterlerinin belirlenmesi tümör anatomisi, invazyon derecesi, çevre dokularla ilişkisi, lenfatik yayılım ve uzak organ metastazları saptanmasına bağlıdır. Endorektal US (EUS) rektum tümörünün duvar içi, perirektal alan ve yakın Iezfatik invazyonlarının gösterilmesinde en duyarlı yöntemdir. İşlem su yastıklı rektal 7.5 Mhz transduser aracılığıyla gerçekleştirilmiş evrelemede de AstlerColler sistematiği kullanılmıştır. Değerlendirmeyi tümörün rektum tabakalarına invazyonu ve nodal yayılım oluşturmuştur. %48 olguda C2 evre saptanmıştır. Evrelemede en önemli overstaging nedeni understaging riski nedeniyle metastatik olarak kabul edilen bazı lenf nodlarının reaktif hiperplazik olmalarıdır. EUS değerlendirme patolojik evreleme karşılaştırmasında lokal invazyon saptamada %81 nodal tutulum tayininde %63 doğruluk oranı saptanmıştır. EUS nin tümör saptamasında büyük ölçüde, nodal tutulumda orta derecede güvenilir bir yöntem olduğu düşünülmektedir.
Abstract
In this study, endorectal sonography was performed to 30 patients diagnosed with rectal carcinoma and hospitalized at IADH. In rectal carcinomas, the choise for appropriate surgery or identification of inoperablity criteria is correlated with tumor anatomy, degree of invasion, nearorgan relationships, lymphatic invasion and for organ metastasis. EUS is the most sensitive tecnique in identifying the invasion of the tumor is the rectal wall, perirectal space and near lymphatic system. The procedure was performed with a 7.5 Mhz rectal waterpedded transducer and the Astler-Coller Gasification was used in staging. The evaluation was performed according to invasion of rectal layers and nodal'status. 48% patients were staged as C2. In staging, the most important reason of overstaging was the acceptance of some reactively hiperplastic lymph nodes as metastatic for risk of understaging. When EUS was correlated with pathologic staging for the determination of local invasion 81% accuracy was found and 63% for nodal status determination. EUS is a highly accurate method in the diagnosis of tumor and highly accurate method for the determination of nodal status.
