Özet

Kliniğimizde yatan bir olgu nedeniyle gebeliğin İntrahepatik kolestazı incelendi. İzmir Atatürk Eğitim Hastanesi 1. Kadın Doğum Kliniğinde yatan 23 yaşında, primigravid bulgularına göre 35 haftalık sarılıklı gebenin karaciğer fonksiyon testleri araştırıldı. Total bilirubin: 10.75 mg/dl, direkt bilirubin: 6.88 mg/dl, SGOT: 45 U/L, SGPT: 60 U/L, ALP: 262 U/L, total protein: 4.3 gr/L düzeylerindeydi. Olgu, hastanemize başvurusundan 30 dakika sonra 1900 gr. ağırlığında, canlı erkek bebek doğurdu. Bebek postpartum 14. saatte mekonyum aspirasyon sendromu tanısı ile kaybedildi. Ancak postpartum ikinci haftada hastanın ikteri kayboldu ve karaciğer fonksiyon testleri normale indi. Gebeliğin İntrahepatik kolestazı, prematurite, ölü doğum gibi yüksek oranda fetal konıp-likasyonlara yol açması nedeniyle antenatal takipte pruritisli olgularda anımsanması gereken bir hastalıktır.

Abstract

An admitted case of intrahepatic cholestasis of pregnancy was discussed in this paper. The liver enzyme levels Of 23 years old primigravid patient with jaundice, who had 35 weeks of pregnancy in ultrasound examination was investigated in first obstetrics and Gynecology Clinic of Atatürk teaching Hospital. The blood values of the patient were; total bilirubin: 10.75 mg/dl, direct bilirubin: 6.88 mg/dl, SGOT: 45 U/L, SGPT: 60 U/L, ALP: 262 U/L, total protein: 4.3 gr/L. 30 minutes after the administration of the patient to the hospital, the patient gave birth to a 1900 gr. male newborn baby. 14 hours after the birth, the newborn died because of meconium aspiratiom syndrome. It took almost two weeks before the patientis jaundice dissapeared and the liver enzyme levels returned to normal levels. During the antenatal clinics, intrahepatic cholestasis of pregnancy should come to mind especially in patients with pruritis; it causes high rates of fetal complications like prematurity or intrauterine fetal deaths.
