Özet
Epstein- Barr virus (EBV) veya Human Herpes Virus tip 4 (HHV-4), dünya nüfusunun %95'ten fazlasını infekte eden bir gama herpesvirustur. Adölesan ve genç eriskinlerde bu virus ile primer infeksiyonun en sık klinik formu kendi kendini sınırlayan bir klinik sendrom olan infeksiyöz mononükleozdur (IM). Çocukluk çagında ise EBV ile infeksiyon genellikle asemptomatik veya hafif seyirlidir. Immun trombositopenik purpura (ITP) çocuklarda sıklıkla kendi kendini sınırlayan bir hastalıktır. Immun sistemin uygunsuz bir cevabı olarak bir viral infeksiyonu veya immunizasyonu takip edebilir. Yüksek ates, bogaz agrısı ve halsizlik yakınmaları ile hastaneye basvuran 21 yasındaki kadın hastada, trombositopeni nedenleri arastırılırken IM tanısı kondu. Infeksiyöz mononükleoza baglı ITP düsünülerek prednizolon tedavisine alındı. Tedaviye çok iyi cevap veren olguda bir yıllık izlemde nüks saptanmadı.
Abstract
Epstein-Barr virus (EBV) or Human Herpes Virus-4 (HHV-4) is a gamma herpesvirus that infects more than 95% of the world's population. The most common manifestation of primary infection with this organism is acute infectious mononucleosis, a self–limited clinical syndrome that most frequently affects adolescents and young adults. Infection with EBV in younger children is usually asymptomatic or mild. Immun thrombocytopenic purpura (ITP) in children is usually a self-limiting disorder. It may follow a viral infection or immunisation and is caused by an inappropriate response of the immune system. In a 21-year old female patient, who admitted to the hospital with the complaints of fever, sore throat and malaise, during the studies for the etiology of thrombocytopenia, infectious mononucleosis was diagnosed. For the treatment of ITP due to infectious mononucleosis, prednisolone was used. The response to the treatment was very good and no relapse was seen in the patient in one-year of follow-up.
