Gergin assitli siroz hastalarında dextran-70 eşliğinde yapılan total parasentezin etki ve komplikasyonlar yönünden değerlendirilmesi ve human albumin eşliğinde yapılanla karşılaştırılması

Özet
Bu çalışma dextran-70 eşliğinde yapılan terapötik parasentezin potansiyel komplikasyonlar yönünden ' güvenilirliğini araştırmak ve aynı amaçla kullanılan human albuminle karşılaştırmak üzere planlandı. Çalışmaya karaciğer sirozu tanısı konan, sodyumdan kısıtlı diyet ve diüretik tedavisine cevap vermeyen gergin assitli 32 hasta alındı. Hastalar randomize olarak iki gruba ayrıldı. 12 hastadan oluşan birinci gruba parasentez esnasında human albumin solüsyonu (6gr/L alınan assit), 20 hastadan oluşan ikinci gruba ise Dextran-70 (6ğr/L alman assit) verildi. Hastaların parasentezden önce ve sonra arteriyel kan basıncı, nabız ve ateş takibi yapıldı. Parasentezden önce, 48 saat ve 6 gün sonra vücut ağırlıkları, serum alanin transaminaz (ALT), aspartat transaminaz (AST), alkalin fosfataz (ALP), protrombin zamanı (PTZ), serum bilirubinleri, serum proteini, üre, kreatinin ve serum elektrolitleri (Na,. K) kontrol edildi. Parasentezden önceki ve sonraki değerler istatistiksel olarak karşılaştırıldı ve aralarında anlamlı bir fark bulunmadı (p>0.05). Her iki grupta da gerek parasentez sırasında, gerekse parasentezden sonraki 6 günlük izlem boyunca herhangi bir komplikasyonla karşılaşılmadı. Sonuçlar bize plazma volüm genişletici olarak dextran-70'in human albumin kadar güvenilir olduğunu gösterdi.
Abstract
The evaluation of the effects and complications of total paracentesis accompanied by dextran-70 and its comparison with human albumin In this study, we evaluate the potential complications and safety of total paracentesis accompanied by dextran-70 as a volume expander and compared it with human albumin. Thirty-two cirrhotic patients, with tense ascites, refractory to sodium-restricted diet and diuretics, were included. The patients were randomly divided into two groups. Human albumin (6 grams per liter) was given to the first group consisted of 12 patients (8 women and 4 men). Dextran-70 (6 grams per liter) was given to the second group consisted of 20 patients (9 women and 11 men). All patients underwent physical examination, blood pressure, temperature and pulse measurements and data concerning the blood biochemical parameters (serum transaminase, alkaline phosphatase, bilirübine, proteine, creatinine, electrolytes) were obtained before paracentesis and 2 and 6 days after procedure. The findings were compared statistically. When the searched parameters were compared, there was no significant difference statistically between before and after paracentesis in the two groups (p>0.05). The results of this study show thet dextran-70, used as a volume expander accompanied by total paracentesis, is as useful as human albumin.
