Bir keçide idiopatik diabetes mellitus

Özet

Üç buçuk yaşlı keçi zayıflık, iştahsızlık, öksürük, kilo kaybı ve kannda şişlik şikayetleri ile kliniğimize getirildi. Genel muayene sonrası laboratuar analizleri için kan ve idrar örnekleri alındı. Total eritrosit (8.2x $10^6$ /mm3) değeri sınırda olan hastanın, kan serum biyokimyasında açlık kan şekeri 17.8 mM/L, amilaz düzeyi 370 .U/L, lipaz düzeyi 384 U/L, fruktozamin 2.39 mmol/L ve açlık serum insulin düzeyi 4.9 $mu$ IU/ml olarak bulundu. İdrar muayenesinde proteinüri (2+) yanında idrarda glikozüri (4+) ve ketonüri (2+) saptandı. Spontan idiopatik diabetes mellitus tanısı konulan keçi hospitalize edildi. Hastaya sıvı ve elektrolit replasyonu yapılarak, eksojen insulin (Monotard, Squib-Novo) tedavisine başlandı. Mastitis, bronkopnömoni ve üriner infeksiyon bulguları mevcut olan ve genel durumu gittikçe bozulan hasta hospitalizasyonun 24. gününde agoni halinde bulunarak kesildi. Otopsisinde verminöz pnömoni, karaciğer yağlanması, pankreas atrofisi ve böbreklerde proliferatif glomerulonefritis bulguları saptandı.

Abstract

A 3.5- years- old goat that was came to our clinic with complains of weakness, anorexia, caughing, polyuria and distention in the abdomen. After general examination, the blood and urine samples were taken for the labaratory analysis. While the erythrocytes (8.2x $10^6$/mm3) was in normal limits, the fasting blood sugar was 17.8 mM/L, amylase was 370 U/L, lipase was 384 U/L, fructosamine was 2.39 mM/L and fasting blood insulin level was 4.9 $mu$ IU/L in blood serum chemistry. In the urine analysis, proteinuria (2+), glikozuria (4+) and ketonuria (1+) were found. Idiopathic diabetes mellitus was diagnosed and she was hospitalised. The fluid and electrolytes replacing were made and exogenous insulin therapy (Monotard insulin, Squibb-Novo) was begun. She also has got mastitis, bronchopneumonia and urinary infection and been going worse, she that was in agony at the 24th of hospitalization and cut. In the necroscopy, verminous pneumonia, fatty liver, pancreas atrophy and proliferate glomerulonephritis were found.
