Özet

Bu çalışmada renal transplantasyon sonrası, geç dönemde değişik endikasyonlarla allograft biyopsisi yapılmış olan 86 hasta incelenerek klinik ve patolojik bulgular karşılaştırılmışım Biyopsi endikasyonu olarak serum kreatinin düzeylerinde gözlenen yükselmeler 58 olgu ile ilk sırayı alırken, bunu 17 olgu ile nefrotik proteinüri ve kreatinin düzeyi yükselmeleri olan grup, 11 olgu ile de yalnızca nefrotik proteinüri grubu izlemiştir. Biyopsi Konuşlarında ise en sık karşılaşılan bulgu 65 hastada saptanan kronik rejeksiyon olmuş, bunu sıra ile 11 olgu ile akut rejeksiyon, 3 olgu ile Cyc A toksisitesi, 3 olgu ile nüks glomerulonefrit ve l olgu ile de yaygın böbrek nekrozu izlemiştir. Biyopsi öncesi klinik ön tanılarının 15 (%17.4) olguda değiştiği saptanmıştır. Biyopsi verilerine göre toplam 63 (%73) olguda tedavi değişikliği yapılmıştır. Sonuç olarak, renal tranplantasyon sonrası geç dönemde karşılaşılan allograft disfonksiyonlarmda radikal tedaviler yapılmadan önce allograft biyopsisi yapılmasının gerekli olduğu ve biyopsinin tedavi seçimi ve prognozu önemli ölçüde etkilediği kanısına varılmıştır.

Abstract

The importance of biopsy in the allograft dysfunction .In the late posttransplant period In this study, 86 renal transplant recipients who had allograft biopsy for different indications were reviewed and clinical and patnological data were compared. The most frequent biopsy indication was elevation in serum creatinine levels in 58 patients, nephrotic range proteinuria and elevated serum creatinine levels in 17 patients and nephrotic range proteinuria alone in 11 patients. The biopsy results were chronic rejection in 65, acute rejection in 11, CycA nephrotoxicity 3, recurrent glomerulonephritis in 3, de-novo glomerulonephritis in 3 and severe renal necrosis in one patients. The clinical and morphological diagnosis were different 15 (17.4%) of 86 patients. The therapies of 63 (73%) patients were changed according to biopsy results. As a result, allograft biopsy must always be performed before the radical therapy of renal transplant patients with allograft dysfunction in the late period and biopsy results will affect the selection of the therapy and prognosis of the graft.
