Özet

Torakotomi, yolaçtığı çeşitli komplikasyonların yanısıra hücresel ve humoral immüniteyi de etkilemektedir. Bu etkileri araştırmak amacıyla çalışmamızda torakotomi uygulanan 100 hastada lökosit miktarı, lenfosit miktarı ve lenfosit oranı preoperatif olarak ve postoperatif 3. saat, l.,2.,3., ve 45. günlerde tetkik edildi. Ayrıca 25 hastada lenfosit yüzey antijenleri (CD3, CD4, CD8, CD4/CD8, CD 19, CD 16/56) ve 21 hastada ise immünglobulin düzeyleri (IgG, IgA, IgM, IgE) preoperatif ve postoperatif 7. günde araştırıldı. Torakotomi sonrasında lökosit miktarında belirgin bir yükselme, lenfosit miktarında ve oranında belirgin bir düşme saptandı (p 0.2). Sonuç olarak, torakotominin lenfosit sayısında postoperatif dönemde düşmeye neden olarak hücresel immüniteyi baskıladığı, ancak hümoral immünitede herhangi bir değişikliğe yol açmadığı ortaya çıkmaktadır.

Abstract

Thoracotomy influences cellular and humoral immunity beside several complications. In our study, leukocyte count, lymphocyte count and lymphocyte ratio of patients operated by day 3rd day and 5th day. Also lymphocyte surface antigens (CD3, CD4, CDS, CD4/CD8, CD 19, CD 16/56) out of 25 patients and immunoglobulin levels (IgG, IgA, IgM, IgE) out of 21 patients on the preoperative period and postoperative 7th day have been detected. After toracotomy, a marked increasing in leukocyte count; while a marked decreasing in lymphocyte count and lymphocyte ratio have been determined (p 0.2). In conclusion, thoracotomy suppresses cellular immunity by decreasing lymphocyte count in postoperative period, however does not alter humoral immunity.
