Özet

ÖZET Bu çalışmada miyokardiyal iskemiye ilişkin semptomları olmayan ve en az 5 senelik tip l diabetes mellitus anamnezi bulunan 25 hastada sessiz miyokardiyal iskemi (SMl) sıklığı ve mikroalbuminüri ile SMl ilişkisi araştırıldı. Sessiz miyokardiyal iskeminin saptanmasında, üç ayrı düzlemde değerlendirilen eforlu miyokard perfüzyonu (SPECT) yöntemi kullanıldı. İncelenen 25 hastanın 12'sinde (% 48) SPECT ile SMl saptandı. Cinsiyet ve diabet süresi ile SMİ arasında anlamlı bir bağlantı kurulamazken mikroalbuminürisi olan tip I diabetes mellituslu hastalarda SMİ sıklığı anlamlı olarak fazla bulundu. Yalnız efor testi sonuçları ile değerlendirildiğinde ise 2 hastada SMİ bulgusuna rastlandı. Çalışma sonunda tip I diabetes mellituslu hastalarda SMl'nin önemli bir sıklıkta görülebildiği ve mikroalbuminürik hastalarda SMİ sıklığının belirgin olarak arttığı sonucuna varıldı.

Abstract

SUMMARY The investigation of silent myocardial ischemia in type I diabetic patients. In this study, the frequency of silent myocardial ischemia (SMI) and the relationship between the microalbuminuria and SMI were investigated in 25 IDDM patients who did not have cardiac symptoms. Single photon hoton emission computarized tomography was used for determination of the SMI. Silent myocardial ischemia was diagnosed in 12 of 25 patients (% 48). While there were no relationships between sex, diabetic age and SMI, the frequency of SMI was higher in patients with microalbuminuria than others. Regarding the exercise test results; SMI was established in two patients. In conclusion, SMI could be seen quite frequently in patients with IDDM and the frequency of SMI is higher in microalbuminuric patients than non-microalbuminuric patients.
