Özet

Hashimoto tiroiditinde sintigrafik bulgular oldukça değişken olup Graves hastalığı, multinodüler guvatr, hiperaktif veya hipoaktif nodul gibi herhangi bir tiroid hastalığını taklid edebilmektedir. Çalışmamızda Hashimoto tiroiditi tanısı konulan 48 hastanın tiroid sintigrafı bulguları bildirilmiştir. On iki hastada (% 25) diffüz hiperplazi, 20 hastada (% 41.7) multinodüler guvatr ve 16 hastada (% 33.3) soliter nodul tespit edilmiştir. Soliter nodüllerin 3'ü toksik adenom, 9'u hipoaktif ve 4'ü normoaktif karakterlidir. Gerekli olmayan cerrahi girişimlerden kaçınmak için bu hastaların sintigrafı, ultrasonografı, tiroid hormonları ve gereğinde ince iğne aspirasyon biyopsileri dahil olmak üzere klinik muayene ve tiroid antikorlarının mutlaka birlikte değerlendirilmesi gerekmektedir.

Abstract

The scintigraphic findings of Hashimoto's thyroiditis (HT) is highly variable and can mimic any thyroid abnormality such as Graves' disease, multinodular goiter, or even a hyperfunctioning or hypofunctioning nodule. We evaluated scintigraphic findings in 48 patients with HT. Thyroid scintigrams revealed diffuse hyperplasia in 12 patients (25 %), multinodular goiter in 20 (41 %), solitary nodule in 16 patients (33.3 %; toxic adenoma 3, hypoactive nodule in 9 and normoactive nodule in 4 patients). In order to eliminate the unnecessary surgical interventions, our results show these patients should be evaluated by means of physical examination, thyroid autoantibodies, as well as thyroid scan, ultrasonography, throid hormones and fine needle aspiration biopsy when necessary as a combination.
