Açık kalp cerrahisinde heparin kaplı kardiyopulmoner bypass sistemlerinin hemoliz yönünden incelenmesi

Özet
Kardiyopulmoner bypass (KPB) sırasında kanın yabancı yüzeyler ile teması, hemoliz, lökopeni ve koagulopati gibi olumsuz etkilere yol açtığı bilinmektedir. Günümüzde KPB'ın bu etkilerini azaltmak amacı ile biyolojik olarak uyumlu olduğu kabul edilen heparin kaplı sistemler geliştirilmiştir. Çalışmamızda; KPB sırasında heparin kaplı sistem (HKS) ile heparin kaplı olmayan sistem (HKOS) kullanılan onbeşer kişiden oluşan iki grup hasta hemoliz yönünden incelendi. HKOS grubunda tam doz heparinizasyon (4 mg/dl) ile antikoagulasyon ile aktive edilmiş pıhtılaşma zamanı (ACT) 480 saniye üzerinde tutulmasına rağmen, HKS grubunda yarı doz heparinizasyon (2 mg/kg) ile ACT 350-400 saniye arasında tutuldu. Plazma serbest hemoglobin (PSHb) düzeyi KPB sırasında HKOS grubunda anlamlı olarak yüksek seyretti. HKS grubunda PSHb değeri postoperatif değere dönerken, bu evrede HKOS grubunda anlamlı olarak yüksek seyretti ve ancak 48. saatte preoperatif değerlere yaklaştı. Diğer yandan HKS grubunda postoperatif kanama miktarı ve kan trasfüzyonu ihtiyacı anlamlı şekilde düşük bulundu. Sonuç olarak ; heparin kaplı sistemlerin, yarı doz heparinizasyon ile güvenli bir KPB imkanı sağlanmasının yanısıra, hemodiyaliz etkilerinin de daha düşük olması nedeni ile tercih edilebileceği kanaatini uyandırmaktadır.
Abstract
SUMMARY Evalluution of heparin-Coated Cardiopulmonaty bypass Circuits Concerning Hemolysis in Open Heart Surgery. It is known that the contact of blood with foreign surfaces during the cardiopulmonary bypass causes some deleterious effects such as hemolysis, leucopenia and coagulopathy. Today, heparin coated circuits that are accepted biocompatible, have been improved to reduce these effects. In our study, two groups consisting of 15 patients in whom heparin coated and uncoated circuits were used during cardiopulmonary bypass have been evaluated for hemolysis. Full dose heparinisation and ACT exceeding 480 seconds applied to the uncoatedgroup inspite of the half dose heparinisation and ACT between 350-400 second applied to the heparin group. The free plasma hemoglobin level were significantly higher in the uncoated group during the cardiopulmonary bypass. In the heparin coated group, free plasma hemoglobin level returned to the initial values at the 12th postoperative hour while it remained significantly higher in the uncoated group and returned close to the initial levels after 48 hours. On the other hand postoperative bleeding and the need for blood tranfusion were significantly lower in the heparin coated group. As a results, it seems that the heparin coated may be preferred for the lower hemolysis effects and their this sale cardiopulmonary bypass acquired with half dose heparinisation.
