Özet
Mitral kapak (valva atrioventicularis sinistra) kompleksini oluşturan annulus, yaprakçıklar (cuspides), komissürler (commissura), kordal yapılar (chordae tendineae) ve papiller kaslar (musculus papillaris anterior-posterior) sol ventrikül bütünlüğünü ve geometrisini sağlamada sistolik pompa fonksiyonu açısından büyük önem taşır. Bu amaçla İstanbul Üniversitesi İstanbul Tıp Fakültesi Anatomi Anabilim Dalı ile Koşuyolu Kalp ve Araştırma Hastanesi Kardiyovasküler Cerrahi Kliniği tarafından 10 fikse kadavra (sekiz erkek, iki kadın) üzerinde, yerinde ve eksize edilmiş mitral kapak kompleksinde anatomik yapılar araştırılmıştır. Annulus anterior ve annulus posterior ile cuspis anterior ve cuspis posterior serbest kenar uzunluk ve yüksekliği ölçülmüştür. Her iki mitral yaprakçık alanı planimet-rik ölçümle hesaplanmıştır. Kordal dağılım ve papiller kasların sınıflaması saptanmıştır. Yürüttüğümüz morfolojik çalışmalarla ayrıntılı veri toplama ve model geliştirme çabalarının, mitral kapak işlevsel anatomisi ve rekonstrüktif mitral kapak cerrahisine ilişkin kavramları geliştireceği inancındayız.
Abstract
The morphological aspects of the mitral valve and subvalvular apparatus. All aspects of mitral valve (valva atrioventircularis sinistra) complex including the annulus, leaflets (cuspides), commissures (commissura) chordal stuctures (chordae tendineae) and papillary muscles (musculus papillaris anterior-posterior) are so important to attain integrity and proper geometric modelling of left ventricle for sistolic pump function. We conducted a detailed investigation of the anatomic structure of both the intact and excised mitral valve complexes of 10 human hearts from fixed human cadavers (eight male and two female) at Department of Anatomy, Faculty of Medicine of Istanbul University and Koşuyolu Heart and Research Hospital. Anterior and posterior annular lenghts, anterior and posterior leaflet edge lenghts, anterior and posterior leaflet heights were measured. Both leaflet areas were calculated from planimetric measurement. Chordal distribution and papillary muscles classification were recorded. We believe that the goal a more precise data collection and developed modelling will influence our understanding of mitral valve functional anatomy, and concept of reconstructive mitral valve surgery.
