Özet

Bu çalışma, bronşial astmalı hastalarda yavaş salınımlı teofılin preparatları ile uzun etkili beta-2 adrenoseptör agonist salmeterolün etkinliğinin karşılaştırılması amacıyla düzenlendi. Polikliniğimizde kronik bronşial astma tanısı konulan hastalardan randomize olarak seçilen 40 hasta üzerinde çapraz karşılaştırmalı bir çalışma uygulandı. Birinci gruptaki 20 hastaya 15 gün, bir uzun etkili teofılin preparatından 200 mg x 2/gün, II. Gruptaki 20 hastaya yine 15 gün, sal-meterol 50 mcgx2 dozunda verildi. Her iki grupta tedaviden sonra elde edilen zirve akım hızı (peak expiratory flow rate=PEF) değerlerinin, önceki PEF değerlerine göre anlamlı derecede yüksek olduğu görüldü. Hastaların semptomları her iki grupta da belirgin derecede düzeldi. Daha sonra iki grubun ilaçları çapraz olarak değiştirildi. Bu şekilde yine 15 günlük bir tedavi uygulandı. Bu süre sonunda elde edilen PEF değerlerinin, her iki grupta da ilk tedaviden sonraki PEF değerlerine göre anlamlı bir fark göstermediği saptandı. Bu bulguların ışığında, bronşial astmalı hastalarda yavaş salınımlı teofılin ile uzun etkili beta - 2 adrenoseptör agonisti salmeterolün eşit düzeyde etkili olduğu sonucuna varıldı.

Abstract

Comparison of slow release oral teophylline with long acting inhaled b2 adrenergic agonist salmeterol. The purpose of this study was to compare the efficacy of slow release oral teophylline and long-acting inhaled beta-2 adrenergic agonist salmeterol in asthmatic patients. The study design was randomized and crossover. 40 patients with stable asthma were randomized to treatment with either slow release teophylline 200 mg (b.i.d.) or salmeterol 50 meg (b.i.d.). After two weeks of treatment with one drug, the patients were given the alternative drug for another two weeks. At the beginning and the end of each treatment period, the best PEF values and asthma symptomscores of patients have been recorded. At the end of the first treatment, teophylline and salmeterol produced nearly equal increase in mean morning PEF and asthma symptomscore. At the end of second period of treatment, there was no significant difference in the PEF values of both groups with regard to the results obtained at the end of the first treatment period. In conclusion, this study shows that both slow release teophylline and long-acting inhaled beta-2 adrenergic agonist salmeterol are equally effective in the treatment of mild-to moderate chronic asthma.
