Hepatit B virusu infeksiyonunun kronikleşmesinde doku gruplarının rolü

Özet
Türk toplumunda hepatitis B virüsü infeksiyonunun kronikleşmesinde HLA doku gruplarının rolü olup olmadığı araştırıldı. Çalışmaya HBV'ye bağlı kronik hepatit veya karaciğer sirozu tanısı konmuş 20 hasta ile asemptomatik HBsAg taşıyıcılığı saptanan 30 vaka alındı. Herhangi bir karaciğer hastalığı bulunmayan ve HBsAg(-) bulunan 50 vaka kontrol grubunu oluşturdu.Tüm vakaların HLA doku grupları Terasiki'nin mikrolenfositotoksisite deneyi ile tayin edildi. Kronik hepatit ve sirozlu hastalarda DR13 ve DQ sıklığı artmış iken, A24 ve CW1 sıklığı azalmış bulundu. Asemptomatik virüs taşıyıcılarında A2, B8, B13, CW3, DR13 sıklığı artmış bulundu. Tüm kronik HBV infeksiyonu bulunanlarda (asemptomatik taşıyıcı, kronik hepatit ve siroz birlikte alındığında) B8, B13, DR7, DR13, VE DQ3 sıklığı artmış bulundu. Sonuçlar yurdumuzda HLA A24 ve CW1 doku gruplarının kronik hepatit B infeksiyonu riskinde azalma ile, buna karşılık B8, B13, DR13 ve DQ3 doku gruplarının kronik hepatitis B infeksiyonu riskinde artma ile beraber olduğunu göstermektedir.
Abstract
The role of HLA antigens in chronicity of hepatitis B virus infection. To investigate the mechanism underlying the development of chronic infection with hepatitis B virus (HBV) in Turkish population, we studied the role of HLA typing. The study group consisted of 20 patients with HBV-related chronic hepatitis or cirrhosis, and 30 healthy earners of HBV. The control group consisted of 50 normal subjects. HLA typing was performed by Terasaki's microlymhocytotoxi-city method. The frequency of HLA-DR13 and DQ3 were significantly higher in the patients with HBV-related chronic liver disease, but there were significant deficiency of HLA-A24 and CW1. The frequency of HLA-A2, B8, B13, CW3, DR 13 were significantly higher in healthy carriers of HBV. There were increased frequencies of HLA-B8, B13, DR13 and DQ3 in all the patients with chronic HBV infection (healthy carriers and chronic liver disease). These results show that HLA-A24 and Cwl were associated with low risk for HBV-related chronic liver disease in Turkish population, and HLA- B13, B18, DR7, DR13, and DQ3 were associated with high risk for chronic HBV infection.
