Özet
Yaşları 12-89 (ort: 38) arasında değişen 80 hastanın 26' sı (% 32.9) kadın, 54' ü (% 67.5) erkekti. Yaş, cins, etyoloji, görme keskinliği, vertikal ve torsiyonel deviasyon miktarı, bilateralite, anormal baş pozisyonu ve diğer sinirlerin tutulumu gibi değişik faktörler kaydedildi. Preoperatif ve postoperatif izleme periyodu sırası ile 6-58 (ort: 7.3) ay ve 1-140 (ort+: 38.3) aydı. Edinsel dördüncü sinir felcinin sebepleri 30 hastada (% 37.5) travma ve 50 hastada (% 62.5) intrinsik faktörlerdi. Travmatik etyolojili 10 hastaya (% 33.3) ve intrinsik etyolojili 7 hastaya (% 14.0) cerrahi uygulama gerekti. Kalan hastalarda spontan regresyon görüldü. İntrinsik faktörler nedeni ile oluşan dördüncü sinir felçlerinde spontan düzelme daha sık gözlenirken, travma nedeni ile oluşan olgularda cerrahiye duyulan gereksinim daha sık görülmektedir.
Abstract
Eighty patients consisted of 26 (32.5 %) female and 54 (67.5 %) male whose ages ranged between 12-89 (median: 38). Various factors including age, gender, etiology, visual acuity, amount of vertical and tortional deviation at presentation, bilaterality, presence of abnormal head posture and additional cranial nerve palsies were noted. Preoperative and postoperative follow-up periods ranged between 6-58 (mean: 7.3) months and 1-140 (mean:38.3) months, respectively. The cause of acquired fourth nerve palsy was trauma in 30(37.5 %) and intrinsic factors in 50(62.5) patients. Surgical intervention was needed in 10(33.3 %) of the patients with traumatic etiology and 7(14.0 %) of the patients with intrinsic etiology in this series. Remaining patients were accepted as having spontaneous regression. Spontaneous recovery is more common in fourth nerve palsies due to intrinsic factors and the need for surgery is more prevalent after a traumatic event.
