Özet

Kolesterol kristal embolisi sendromu, deri, böbrek, kas, göz, gastrointestinal ve santral sinir sistemini ilgilendiren multisistemik bir hastalıktır. Aorta ve büyük atfederdeki aterom. plaklarından mekanik veya spontan olarak kopan kolesterol kristallerinin, dolaşıma katılarak küçük arterleri tıkaması ve organ hasarına neden olması ile karakterizedir. Kolesterol kristal embolisi, yaşlı hastalarda, anjiografi, vasküler cerrahi, trombolitik ve antikoagülan tedavi sonrası sık gelişebilen bir komplikasyondur fakat tanı konulmasında genellikle güçlüklerle karşılaşılır. İnvaziv arteriyel girişim sonrası, böbrek fonksiyonlarında kötüleşme, hipertansiyon, distal iskemi veya akut multiorgan fonksiyon bozukluğu gelişen hastalarda kolesterol kristal embolisi sendromu mutlaka düşünülmelidir. Yazımızda akut böbrek yetersizliği ve periferik arter embolisi ile kendini gösteren bir kolesterol kristal embolisi vakası tartışılmıştır.

Abstract

Acute renal failure and peripheic arterial embolism due to cholesterol crystal embolism following coronary anjiography, PTCA and"stenting: a case report. Cholesterol crystal emboli syndrome is a multisystemic disorder generally involving the skin, muscle, kidneys, eyes, gastrointestinal tract and central nervous system. It is characterized by occlusion of small arteries with cholesterol crystal emboli deriving from mechanically or spontaneously eroded atherosclerotic plaques of the aorta or larger feeder arteries, causing organ damage. Cholesterol crystal embolism is a common complication of angiography, vascular surgery, thrombolytic and anticoagulant therapy in elderly patients but often, this diagnosis not considered. A patient who develops worsening renal function, hypertension, distal ischemia, or acute multisystemic dysfunction following an invasive arterial procedure should be suspected of having cholesterol embolism syndrome. Thereby, we present a patient with cholesterol crystal emboli who presented with acute renal failure and peripheric embolism after coronary angiography.
