Özet

Amaç: Obezite tüm dünyada giderek artan sağlık sorunlarından biridir. Yağ dokusunun birçok hormon, sitokin ve kimyasalların homeostaz ve regülasyonunda önemli rolü bulunmaktadır. Obezite tüm sistemleri etkilemektedir. Tiroid fonksiyonları da etkilenen sistemlerden biridir. Tiroid hormonlarının adaptif termogenezis, istirahat enerji gereksinimi üzerinde belirleyici etkileri vardır. Bu nedenle obezite ile tiroid fonksiyonları arasında belirgin bir korelasyon beklenir. Biz de obezite ile tiroid fonksiyonları arasındaki ilişkiyi incelemeyi amaçladık.Yöntem: Yüz altmış iki obez olgunun (21 erkek, 141 kadın) antropemetik ölçümleri ile T4, tiroid stimulan hormon (TSH) değerlerine bakıldı.Bulgular: Hastaların obezite yaşları 16,6±11,8 idi. Vücut kitle indeksi (VKİ): 39,69±6,06, bel çevresi: 112,80±12,72 cm bulundu. T4: 1,31±0,19 ng/dl, TSH: 2,70±2,63 mU/dl idi. İki hastada hipertiroidi tespit edildi. Hipotiroidik hasta görülmedi, 23 hastanın TSH seviyeleri yüksek bulundu.Sonuç: Genelde obezlerde T3-T4 seviyeleri normal sınırlarda olmasına karşın hastaların %15’ inde hafif artmış TSH seviyeleri tespit edildi. TSH ile VKİ arasında pozitif korelasyon vardı.

Abstract

Objectives: Obesity is a major health problem, and prevalence of obesity is increasing worldwide. Various hormones, cytokines and chemokines secreted by adipose tissue exert important roles and regulation in homeostasis. Obesity affects all systems. Thyroid function is affected by obesity. Resting energy expenditure and adaptive thermogenesis are determined by thyroid hormone. We aimed to investigate the association between obesity and thyroid function.Methods: Anthropomorphic values and thyroid hormone levels were measured in a group of 162 obese patients (21 male, 141 female).Results: Duration of obesity in patients was 16.6±11.8 years. Anthropomorphic and hormone values were as follows: body mass index (BMI): 39.69±6.06, waist circumference: 112.8±12.72 cm, T4: 1.31±0.19 ng/dl, and thyroid-stimulating hormone (TSH): 2.70±2.63 mU/dl. There were 2 hyperthyroid patients and no hypothyroid patient. Elevated TSH levels were confirmed in 23 patients.Conclusion: Although we found normal serum T3 and T4 values in obese subjects, 15% of patients had slightly high TSH levels. There was a positive correlation between TSH and BMI.
