Özet

Amaç: Pegileinterferon veya nükleozid analoglarıyla viro- lojik ve biyokimyasal yanıt elde edilen olgularda alevlen- me hepatiti gelişimini belirleyen değişkenleri incelemektir. Gereç ve Yöntem: Bu çalışma hepatit B virüs (HBV) enfek- siyonlu olguların verilerini değerlendiren geriye dönük ke- sitsel bir çalışmadır. Hasta dosyalarından olguların boy, kilo,cinsiyet, yaş, alkol kullanım durumu, tedavi öncesi HBVDNA ve ALT düzeyi, Knodell skorlama sistemine göre öl- çülen histopatolojik aktivite indeksi ve fibroz skoru, tedavisüresi, tedaviye biyokimyasal ve virolojik yanıt süresi, HBe- Ag durumu, kullanılan tedavi türü ve alevlenme hepatiti ge- lişip gelişmediği öğrenildi. Hastalar alevlenme hepatiti olanve olmayanlar olarak iki gruba ayrıldı. İki grup değerlendir- meye alınan değişkenler açısından karşılaştırıldı. Bulgular: Çalışmamıza 18 (%30) kadın, 42 (%70) erkek ol- mak üzere toplam 60 hasta alındı. Altmış hastanın 25’inde(%41) tedavi sonrası alevlenme hepatiti geliştiği görül- dü. Alevlenme hepatiti gelişen hastaların tedavi sürelerininalevlenme hepatiti gelişmeyen hastaların tedavi sürelerinegöre istatistiksel olarak anlamlı ölçüde daha uzun olduğu (sı- rasıyla ortalama 22 ay ve 18 ay, p=0,029) görüldü. Lamivu- din kullanan 42 hastanın 18’inde (%42) ve adefovir tedavi- si alan 7 hastanın tümünde (%100) alevlenme hepatiti geli- şirken interferon kullanan 11 hastanın hiçbirinde (%0) alev- lenme hepatiti görülmedi. Alevlenme hepatiti adefovir alanolgularda, lamivudin ve interferon alan olgulara göre dahasıktı (p=0,010). Alevlenme hepatiti lamivudin alan olgular- da interferon alan olgulara göre daha fazlaydı (p=0,010). Sonuç: Alevlenme hepatiti uzun tedavi süresi olanlardadaha sık görülür. Adefovir kullananlarda lamivudin ve in- terferon kullananlara göre alevlenme hepatiti daha sıktır.Lamivudin kullananlarda alevlenme hepatiti interferon kul- lananlara göre daha sıktır.

Abstract

Objectives: We aimed to determine independent variables pre- dictive of flare hepatitis in chronic hepatitis B-infected patientsin whom virologic and biochemical response was obtainedwith nucleoside analogue or pegylated interferon therapy. Methods: This study was planned as a cross-sectional ret- rospective evaluation of data pertaining to chronic hepati- tis B virus (HBV) cases. Patients’ body weight, height, gen- der, age, alcohol consumption, pretreatment serum HBVDNA and alanine aminotransferase levels, histopathologicactivity index according to the Knodell scoring system, fi- brosis score, treatment duration, biochemical and virolog- ic response time to treatment, HBeAg status, presence offlare hepatitis, and drugs used in the HBV treatment wereobtained from case files. Patients were divided into twogroups as with and without flare hepatitis. These groupswere compared in terms of the variables being assessed. Results: Sixty patients (18 female, 42 male) were includedin this study. Flare hepatitis developed in 25 (41%) patients.The treatment durations of patients with flare hepatitiswere longer than of the patients without flare hepatitis (22months and 18 months, respectively, p=0.029). Eighteenof 42 patients treated with lamivudine, all patients treatedwith adefovir and none of 11 patients treated with interfer- on had flare hepatitis. Flare hepatitis was more common inthe patients treated with adefovir than with lamivudine andinterferon (p=0.01), and was more common in the patientstreated with lamivudine than with interferon (p=0.01). Conclusion: We conclude that: 1-Prolonged duration oftreatment increases the flare hepatitis rate, and 2-Flarehepatitis was more common in the patients treated with ad- efovir than with lamivudine and interferon. Furthermore,flare hepatitis was more common in the patients treatedwith lamivudine than with interferon.
