Geriatrik olgularda akut kolesistit tedavisinde ultrasonografi kılavuzluğunda perkütan kolesistostominin yeri

Özet

AMAÇ: Akut kolesistitli geriatrik olgularda ultrasonografi kılavuzluğunda perkütan kolesistostominin (PK) sonuçlarını değerlendirmek.YÖNTEMLER: Akut kolesistit nedeniyle PK uygulanmış olan yaşlı olgular (65 yaş ve üzeri) olgular retrospektif olarak tarandı. PK, ko-morbiditeler ya da yüksek Amerikan Anesteziyoloji Derneği (ASA) skorlarına bağlı yüksek cerrahi riski olan olgularda uygulanmıştı. Akut ataktan iyileştikten sonra, olgular interval kolesistektomi açısından değerlendirildi. BULGULAR: Çalışmaya 12 olgu (7 kadın, 5 erkek) dahil edildi. On olgu hastaneye akut kolesistit ile başvurmuş iken, 1 olguda endoskopik retrograd kolanjiyopankreatografi sonrasında, 1 olguda ise başka nedenlere bağlı hastane yatışı esnasında akut kolesistit gelişmişti. PK öncesinde yapılan ultrasonografik değerlendirmede hidropik safra kesesi ve safra kesesi duvarında kalınlaşma tüm olgularda görülürken, olguların 7’sinde kolesistolityazis, 9’unda safra çamuru, 3’ünde perikolesistik sıvı ve 1’inde perikolesistik apse saptandı. İşlemin teknik başarısı ve klinik iyileşme %100 idi. İşleme bağlı ve 30 günlük mortalite görülmedi. Komplikasyonlar (vagal reaksiyon ve hepatik kolleksiyon), dört (%40) olguda görüldü. İki olguda akut atak iyileştikten sonra interval kolesistektomi uygulandı. Cerrahi uygulanmayan olgularda kısa dönem takipte akut kolesistit rekürrensi görülmedi.SONUÇ: Ultrasonografi kılavuzluğunda PK, geriatrik olgularda akut kolesistit tedavisinde başarılı ve güvenli bir yöntemdir. Elektif cerrahi, hastaların klinik durumları düzeldiğinde daha düşük riskle uygulanabilir.

Abstract

OBJECTIVE: To evaluate the results of ultrasound guided percutaneous cholecystostomy (PC) in geriatric patients with acute cholecystitis. METHODS: Elderly patients (65 years of age or older) who underwent PC for acute cholecystitits were retrospectively reviewed. PC had been performed in patients with high surgical risks due to co-morbidities or high American Society of Anestesiology (ASA) scores. Patients were evaluated for interval cholecystectomy after the acute attack was resolved. RESULTS: Twelve patients (7 female, 5 male) were included into the study. Ten patients presented to the hospital with acute cholecystitis while acute cholecystitis developed following endoscopic retrograde cholangiopancreatography in 1 patient and during hospitalization for other morbidities in 1 patient. Ultrasonographic evaluation before PC revealed gallbladder hydrops and thickening of the gallbladder wall in all patients, while cholecystolithiasis in 7 patients, gallbladder sludge in 9 patients, pericholecystic fluid in 3 patients, and pericholecystic abscess in 1 patient were also found. Technical procedure success and clinical improvement was achieved in 100% of patients. Procedure related and 30-day mortality did not occur. Complications (vagal reaction and hepatic collections) were seen in 4 (40%) patients. Interval cholecystectomy was performed in two patients after the acute attack was resolved. Patients who did not undergo surgery did not show recurrence of acute cholecystitis during short- term follow up. CONCLUSION: Ultrasound guided PC is a successful and safe procedure in treatment of acute cholecystitis in geriatric patients. Elective surgery can be performed with lower risk rates once the clinical condition improves.
