Özet
AMAÇ: Çalışmamızda hastanemizde uygulanan çeşitli invaziv girişimler sonrası meydana gelen iatrojenik pnömotoraksların etyolojileri araştırıldı. YÖNTEMLER: Eylül 2006- Aralık 2011 tarihleri arasında hastanemizin farklı bölümlerinde yatan ve iatrojenik pnömotoraks gelişen 27 olgu geriye dönük olarak incelendi.BULGULAR: Olguların 11’i kadın, 16’sı erkek, yaş ortalamaları 44 (19-74) idi. En sık iatrojenik pnömotoraks nedeni santral venöz kateter uygulaması (n: 8) % 29.6 idi. İkinci en sık neden torasentez (n: 6) %22.2, üçüncü en sık neden ise transtorasik ince iğne aspirasyon biyopsisi (n.5) %18.5 idi. Tedavi yöntemi 8 olguda nazal oksijen ile gözlem (pnömotoraks derecesi %20’nin altında), 6 olguda basit aspirasyon, 12 olguda toraks tüpü, 1 olguda cerrahi tedavi (hava kaçağı >7 gün) idi. SONUÇ: İatrojenik pnömotoraks, fark edildiğinde tedavisi mümkün olan ciddi bir komplikasyondur. Uygun teknik ve tecrübe, iatrojenik pnömotoraks riskini azaltacağı kanaatindeyiz.
Abstract
Objectives: We investigated the etiologies of iatrogenic pneumothoraces that may occur after various invasive pro- cedures performed at our hospital.Methods: The charts of 27 patients were retrospectively reviewed from September 2006-December 2011. Results: 11 patients were female and 16 were male, with a mean age of 44 (19-74) years. The most common cause of iatrogenic pneumothorax was central venous catheter insertion (n: 8, 29.6%), followed by thoracentesis (n: 6, 22.2%), and transthoracic needle aspiration biopsy (n: 5, 18.5%). Treatment choices for these cases were as follows: observation with nasal oxygen for 8 (pneumothorax degree below 20%), simple aspiration for 6, chest tube application for 12, and surgery for 1 patient(s) (air leakage >7 days). There was no mortality. Conclusion: Iatrogenic pneumothorax is a serious compli- cation that is treatable if it is noticed. We believe proper technique and experience reduce the risk of iatrogenic pneumothorax.
