Özet

Çalışmamızın amacı, Gram-pozitif (Gram+) veya Gram-negatif (Gram-) bakterilerin etken olduğu sepsis hastalarının serum C-reaktif protein (CRP) ve prokalsitonin (PCT) düzeylerinin, kan kültürü sonuç- larından çok daha erken olarak, Gram+/Gram- ayırıcı tanısında kullanılıp kullanılamayacağını belirlemektir. Yöntemler: Şubat 2014 - Şubat 2016 tarihleri arasında kan kültürü pozitifliği ile birlikte klinik olarak bakteriyel sepsis tanısı almış olan 47 vaka retrospektif olarak çalışmaya dahil edildi. 15 Gram+ ve 32 Gram- bakterinin etken olarak izole edildiği sepsis gruplarında CRP ve PCT düzeyleri arasında anlamlı bir fark olup olmadığını tespit etmek için Mann-Whitney U testi yapıldı. CRP ve PCT düzeyleri arasındaki korelasyon Spearman testi ile değerlendirildi. Bulgular: Tüm bakteriyel sepsis vakalarının medyan CRP değeri 91,42 mg/L, medyan PCT değeri de 0,46 ng/mL olarak bulundu. Gram+ ve Gram- sepsis grupları arasında CRP ve PCT düzeyleri arasında istatistiksel olarak anlamlı bir fark bulunmadı (sırasıyla, p=0,98 ve p=0,21). Abstract

The primary aim of our study was to investigate theusefulness of serum C-reactive protein (CRP) and procalcitonin(PCT) levels in the differential diagnosis of causative gram-positive(Gram+) or gram-negative (Gram-) bacteria in patients with sepsisto facilitate decisions concerning the initial choice of an empiricantibiotic regimen.Methods: Between February 2014 and February 2016, 47 patientswho had sepsis diagnosed on the basis of positive blood culturesand clinical examination results were retrospectively enrolled. SerumCRP and PCT levels of 15 gram+ and 32 gram- bacterial sepsisgroups were compared using the Mann–Whitney U test. The correlationbetween CRP and PCT levels was calculated using the Spearman’stest.Results: Among patients with bacterial sepsis, the median CRP levelwas 91.42 mg/L and the median PCT level was 0.46 ng/mL. There wereno significant difference in CRP and PCT levels between the gram+and gram- sepsis groups (p=0.98 and p=0.21, respectively). There wasgood correlation between the CRP and PCT levels (r=0.64, p
