Auricular Helix Carcinosarcoma; an Aggressive Tumor Characterized: Case Report

Özet
Primer kutanöz karsinosarkom, epiteliyal ve mezenşimal komponentleriolan sıklıkla vücüdun güneş ışığına maruz kalan baş boyun ve üst ekstremitegibi bölgelerinde görülen bifazik bir tümördür. Daha çok yaşlıerkeklerde görülür ve oldukça nadirdir. Günümüzde histogenezisi halatartışmalı olan bu tümörün tanısı immunhistokimyasal değerlendirmeile konur. Tümörün epiteliyal komponentleri daha çok squamöz hücrelikarsinom, bazal hücreli karsinom ve adneksal tümörler iken, mezenşimalkomponentleri pleomorfik sarkoma, atipik fibroksantoma ve osteosarkomadır.Literatürde çok az sayıda bildirilen aurikula heliks karsinosarkomvakalarına katkıda bulunmak amacıyla bir yıl önce aurikula heliksindentotal eksize edilen karsinosarkom olgusunun parotis metastazını sunuyoruz.Hastaya total parotidektomi ve modifiye radikal boyun diseksiyonutip 2 yapılmıştır. Hastanın genel durumu kemoterapi alması için uygunolmamıştır. Postoperatif 25 seans adjuvan radyoterapi yapılan hastanıntakiplerinde tümör agresif bir seyir göstermiş ve altıncı ay içinde temporomandibularbölgede rekürrens saptanmıştır. Bunun üzerine hastaya 25seans daha radyoterapi uygulanmış ve hasta 11. ayda eks olmuştur
Abstract
Primary cutaneous carcinoma is a dermal biphasic tumor that is morecommonly encountered in the upper extremities and the head and neckregion, as a result of sunshine exposure. It is a rare tumor and occursmore frequently in older males. Immunohistochemical evaluation is requiredfor the diagnosis. The epithelial components of the tumor includesquamous cell carcinoma, basal cell carcinoma and adnexial tumors; mesenchymalcomponents are pleomorphic sarcoma, atypical fibroxanthomaand osteosarcoma. We present a rare case where auricular helix carcinosarcomametastasized to the parotid gland within the first year followingtotal excision. The patient had a total parotidectomy as well as a modifiedradical neck dissection type II and postoperative adjuvant radiotherapy. Inspite of the surgery and radiotherapy, the tumor had an aggressive course,with recurrence in the temporomandibular region, and the patient waslost on the 11th month of the follow-up
