Diyabetik ve Diyabetik Olmayan Koroner Arter Hastalarında Fibrinojen ve Ortalama Trombosit Hacminin Aterosiklerozdaki Yeri

Özet
Amaç: Koroner arter hastalığı olmayan tip 2 diyabetik hastalarda ortalama trombosit hacmi (OTH) ve fibrinojen düzeyleri ile HbA1c, total kolesterol, low density lipoprotein (LDL)-Kolesterol, high density lipoprotein (HDL)-Kolesterol, trigliserit ve açlık kan şekeri düzeyinin kardiovaskuler komplikasyon gelişimindeki önemini araştırmak ve bu risk faktörlerinin birbirleriyle olan ilişkilerini karşılaştırmayı amaçladık. Yöntemler: Çalışmada 1.1.2009-31.12.2011 tarihlerinde İstanbul Eğitim ve Araştırma Hastanesi Diyabet Polikliniğine başvuran ve takip edilen Tip 2 diyabetes mellitus ve koroner arter hastalığı (KAH) olan 182 hastanın dosyaları retrospektif olarak incelendi. Olguların yaşları ortalama 63,07±11,24 (40-90) ve 115'i erkek (%63,2), 67'si (%36,8) kadındı. Hastalar 3 grupta incelendi. Grup 1: KAH olan ve Diabetes Mellitus (DM) olmayan hastalar (n=59), Grup 2: KAH ve DM olan hastalar (n=60), Grup 3: DM olan ve KAH olmayanlar (n=62). Bulgular: Grupların ortalama yaşları sırasıyla; 64±13, 62±9, 57±10 yıl idi. Post hoc analizde Grup 3'ün grup 2 ve 1'e oranla fibrinojen ve OTH değerleri anlamlı olarak düşük saptandı. Gruplar arasında total kolesterol, LDL kolesterol ve HbA1c ortalama değerleri ile açlık kan şekeri (AKŞ) ve trigliserit ortanca değerlerinde istatistiksel önemde anlamlı farklı saptandı. HDL kolesterol ortalama değerlerinde gruplar arasında anlamlı farklılık görülmedi. Sonuç: OTH ve fibrinojenin koroner arter hastalığı olmayan Tip 2 diyabetik hastalarda kardiyovasküler komplikasyon gelişimi için bağımsız risk faktörleri olabileceği ve hastaların koroner arter hastalığı için takibinde kullanılabilecek bir belirteç olarak kabul edilebileceği düşünülmüştür.
Abstract
Objective: The aim of this study is to determine the importance of factors, such as mean platelet volume (MPV), fibrinogen, HbA1c, total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, and fasting glucose, on cardiovascular complication processes for type 2 diabetes mellitus patients without coronary artery disease (CAD) and also to compare the relationship between these risk factors. Methods: For this study, data of 182 patients who with coronary hard disease and diabetes mellitus were retrospectively analyzed. The patients were followed up at the Outpatient Clinic of Diabetes of Istanbul Education and Research Hospital between 01.01.2009-31.12.201.The mean age of patients were 63.07±11.24 (40-90) years. 115 (63.2%) patients were male and 67 (36.8%) patients were female. The patients were divided into three groups: group 1: patients who had CAD but no DM (n=59), group 2: patients who had CAD and DM (n=60), and group 3: patients who had DM but no CAD (n=62). Results: 201 The mean age of patients was 63.07±11.24 years; 115 patients were male and 67 were female. The patients were divided into three groups: group 1: patients who had CAD but no DM (n=59), group 2: patients who had CAD and Diabetes Mellitus (DM) (n=60), and group 3: patients who had DM but no CAD (n=62). The mean age of patients in each group was as follows: 64±13, 62±9, and 57±11 years. According of post hoc analyses, the fibrinogen and MPV values of group 3 were lower than group 1 and group 2. There was a statistical difference between the groups in terms of mean values of total cholesterol, LDL-cholesterol, HbA1c, and the median values of fasting glucose and triglyceride. There was no difference between the 3 groups in terms of mean values of HDL-cholesterol. Conclusion: In conclusion, MPV and fibrinogen values may be independent risk factors of cardiovascular complications for type 2 DM patients and may be suggested as useful markers to follow up patients for progression to coronary artery disease.
