Unruptured Live Ectopic Pregnancy at 11 Weeks of Gestation

Özet

Ektopik gebelik, fertilize olmuş ovumun uterin kavite dışında herhangi bir yerde implantasyonu şeklinde tanımlanır. Ektopik implantasyonun en sık yerleştiği bölüm, fallopian tüplerdir. Tanı klinik olarak vajinal kanamanın olduğu veya olmadığı amenoreye eşlik eden pelvik ağrı ve pozitif gebelik testi ile konulabilir. Ek- topik gebelikler tüm anne ölümlerinin %10-15'ini kapsamaktadır. Dolayısıyla tubal rüptürü oluşmadan önlemek oldukça önemlidir. Erken tanı transvajinal ultrason ve human koryonik gonadotropin seviyesiyle konulabilir. Fakat kesin tanıya cerrahi ve doku örnek- lerinin mikroskopik incelemesiyle ulaşılır. Tubal ektopik gebeliğin esas tedavi şekli cerrahi veya metotreksat ile sistemik medikal tedavidir. 26 yaşında rüptür olmamış fetal kalp atımı mevcut 11 haftalık ektopik gebelik olgusu sunulmaktadır. Laparoskopik sal- penjektomi uygulanmıştır. Ektopik gebelik yaklaşık 11 haftalık cesamette idi. Doku örneklerinin histopatolojik incelenmesi tubal gebeliği desteklemiştir

Abstract

Ectopic pregnancy is defined as the implantation of a fertilized ovum at a site other than the uterine endometrium. The fallopian tube is the most common site for ectopic implantation. Diagnosis is clinically based on a history of pelvic pain associated with amenorrhea and a positive preg- nancy test result with or without slight vaginal bleeding. Ectopic pregnan- cies account for 10%-15% of all maternal deaths. The prevention of tubal rupture before its occurrence is extremely important. An early diagnosis can be made by performing a transvaginal ultrasound examination and measuring human chorionic gonadotropin levels. However, a definitive diagnosis is made after performing a surgery and microscopic examina- tion of tissue specimens. The main treatment options for tubal ectopic pregnancies are surgery or systemic medical treatment with methotrex- ate. We present the case of a 26-year-old woman with unruptured 11 weeks of gestation with fetal heart beat-positive tubal ectopic pregnancy. We performed laparoscopic salpingectomy. A histopathological examina- tion of the tissue specimens revealed tubal ectopic pregnancy
