Abstract
Hypertrophic olivary degeneration (HOD) is a rare form oftranssynaptic degeneration and is the only type which occurswith hypertrophy rather than atrophy. It develops due topathologies affecting conduction in the dentato-rubro-olivarypathway. Despite being the classic HOD-related findings, palataltremor, Holmes tremor, nystagmus, and cerebellar ataxia arenot usually present in patients. Most cases are asymptomatic, and HOD is incidentally detected on magnetic resonanceimaging scans. This paper reports the case and imaging resultsof a 60-year-old male patient that presented with findings ofacute infarction at the posterior aspect of the mesencephalonand pons and superior cerebellar peduncle following headtrauma and developed right contralateral HOD.
