Özet

Amaç: Kliniğimizdeki "Koriyon Villus Örneklemesi"nin (CVS) endikasyonları, sitogenetik analiz ve gebelik sonuç- larını sunmayı amaçladık. Gereç ve Yöntem: 2012-2015 yılları arasında NT >=2,5 mm, ilk trimester kombine testi yüksek riskli, kistik higroma ve ebeynlerden birinde transkolasyon taşıyıcısı olanlar dört alt gruba ayrılıp, CVS yapılan 354 gebe retrospektif olarak ele alındı. Bulgular: Doksan sekiz (%27,68) olguda fetal sitogenetik anormallik saptandı. Otuz dört (%34,69) olgu trizomi 21, 3 (%3,06) olgu trizomi 13, 8 (%8,16) olgu trizomi 18, 14 (%14,28) olgu monozomiydi ve bunların 9 (%64,9)'u kistik higroma nedeni ile CVS yapılan olgulardı ve diğer endikasyonlara göre yüksek olarak belirlendi. Mozaizm, 17 (%17,34) olguda saptandı. İşlem sonrası fetal kayıp oranı 6 (%1,69) ve kültürde üreme başarısızlık oranı 9 (%2,54) idi. Kistik higroma ve NT >=2,5 mm nedeni ile CVS yapılanlarda gebeliğin tahliye ile sonlanma oranı ilk trimester kombine teste göre anlamlı yüksekti (p=0,018). Tartışma ve Sonuç: CVS ilk trimester de prenatal tanı için kullanılan altın standart testtir. CVS endikasyonlarından bağımsız olarak, fetal sitogenetik analiz sonuçlarının çoğu trizomidir. NT kalınlığı ve kistik higroma olanlarda fetal sitogenetik anomali görülme oranı, fetal kayıp oranı ve tahliye oranı artmaktadır.

Abstract

Introduction: We aimed to present indications, cytogenetic analysis and results of chorionic villus sampling (CVS) and we share our experiences with CVS. Material and Methods: We divided the patients who presented to our clinic between the years 2012, and 2015 with NT ≥2.5 mm, highly risky first trimester combined test results, and cystic hygroma whose one their parents was translocation carrier into 4 subgroups, and 354 pregnant women who underwent CVS were investigated retrospectively. Results: Fetal cytogenetic abnormalities were detected in 98 (27.68%) cases. Patients underwent CVS due to trisomy 21 (n=34; 34.69%) , trisomy 13 (n= 3; 3.06%), trisomy 18 (n=8; 8.16%), monosomy, and cystic hygroma (n=9; 64.9%) of monosomy cases with higher frequency when compared with other indications of CVS. Mosaism was detected in 17 (17.34%). Postprocedural fetal loww, and coulture failure were detected in 1.69% (n=6), and 2.54 % of the patients, respectively. Termination of pregnancy rate in patients who underwent CVS because of cystic hygroma, and NT ≥2.5 mm was significantly higher when compared with the combined first trimester test (p=0.018). Discussion and Conclusion: CVS is the gold standard test used for the prenatal diagnosis during the first trimester. The most frequently encountered fetal cytogenetic analysis result is trisomy irrespective of CVS indications. Abnormal fetal cytogentic analysis results, fetal loss and medical evacuation rate increases in patients with thicker NT, and cystic hygroma.
