İntratekal Fentanilin Post Spinal Baş Ağrısı Sıklığı Üzerine Etkisi

Özet

Amaç: Spinal anestezi alt ekstremite cerrahilerinde sıklıkla yeğlenmektedir. Spinal anestezinin hasta konforunu bozan post spinal baş ağrısı gibi çeşitli komplikasyonlar bulunmaktadır. Biz bu çalışmada, heavy marcaine ilave edilen fentanilin post spinal baş ağrısı görülme sıklığı üzerine etkisini araştırmayı amaçladık.Gereç ve Yöntem: Variköz ven cerrahisi planlanan hastalar randomize iki gruba ayrıldı. Kontrol grubuna 15 mg hiperbarik bupivakain, çalışma grubuna 15 mg hiperbarik bupivakain+20 mcg fentanil uygulandı.Bulgular: İki yüz altı hastaya (116 erkek, 90 kadın) ait veri analiz edildi. Post spinal baş ağrısı görülme sıklığı gruplar arasında benzerdi. Post spinal baş ağrısı ile ilişkili olan değişkenlerin multivaryant regresyon analizinde boy, alkol kullanımı, deneme sayısı, girişim öncesi parsiyel oksijen satürasyon değeri ve girişim sonrası 10. dakika kalp hızı post spinal baş ağrısı için bağımsız risk faktörleri olarak saptandı. Sonuç: Spinal anestezi sırasında heavy marcaine ilave edilen 20 mcg fentanilin post spinal baş ağrısı üzerine etkisi bulunmadığını düşünmekteyiz. Ancak alkol kullanımı, lomber ponksiyon sayısı, bazal parsiyel oksijen satürasyon değeri ve girişim sonrası 10. dk. kalp hızı post spinal baş ağrısı için bağımsız risk faktörleri olarak saptandı. Bu değişkenlerle ilgili yeni çalışmalara gereksinim duyulduğu düşüncesindeyiz.

Abstract

Objective: Spinal anesthesia is often preferred for lower extremity surgery. There are various complications related to spinal anesthesia such as post-spinal headache which causes patient discomfort. In this study, we aimed to investigate the effect of fentanyl added to heavy marcaine on the incidence of post-spinal headache. Material and Methods: The patients scheduled for varicose vein surgery were randomized into two groups. Hyperbaric bupivacaine (15 mg) was given to the patients in the control group, where 15 mg hyperbaric bupivacaine and 20 mcg fentanyl were given to the patients in the study group.Results: Data of 206 patients (116 males, 90 females) were analyzed. The incidence of post-spinal headache was similar in both groups. With multivariate regression analysis of variables related to post-spinal headache, height, alcohol usage, the number of trials for injection, partial oxygen saturation before intervention and the heart rate of the patient measured at the 10th minute following the intervention were found as independent risk factors for post-spinal headache. Conclusion: We think that 20 mcg fentanyl added to heavy marcaine during spinal anesthesia had no effect on postspinal headache. However, alcohol usage, the number of lumbar punctures, baseline partial oxygen saturation value and the heart rate of the patient measured at the 10th minute following the intervention were found as independent risk factors for post-spinal headache. We think that there is need for new studies on these variables.
