Kliniğimizin Yüzeyel Parotidektomi Tecrübesi ve Sonuçları: Üç Yıllık Analiz

Özet
Amaç: Çalışmamızın amacı, parotis benign tümörü nedeniyle yüzeyel parotidektomi yaptığımız 12 hastanın retrospektif olarak incelenmesi; pre-operatif değerlendirme, yapılan cerrahi işlemler, post-operatif patolojik sonuçlar, takip edilme süreleri ve komplikasyonların değerlendirilmesidir.Gereç ve Yöntem: Kasım 2013 ile Ekim 2016 tarihleri arasında yüzeyel parotidektomi uygulanmış 12 hastaya ait veriler, hastane arşivinde yer alan hasta dosyalarından ve elektronik kayıtlardan sağlanarak retrospektif olarak incelendi. Hastaların yaşı, cinsiyeti, tümörün tarafı, tümörün boyutu, sigara kullanımı, preoperatif ince iğne aspirasyon biyopsisi (İİAB) sonucu, insizyon şekli, fasiyal sinir monitorizasyon durumu, dren kalma süresi, postoperatif komplikasyonlar, postoperatif patoloji sonucu, takip süresi, ameliyat tekniği kaydedildi.Bulgular: Yüzeyel parotidektomi (YP) yapılan 12 hastanın 7'si kadın (%58.3), 5'i erkek (%41,7) idi. Olguların ortalama yaşı 42,2 idi. Hastaların yapılan İnce İğne Aspirasyon Biyopsisi (İİAB)'sinde 8'i (%66.6) pleomorfik adenoma veya mikst epitelyal hücre, 2'si (%16.7) Whartin tümörü ön tanısı alırken 2'sinde (%16,7) ise nondiagnostik olarak değerlendirildi. Postoperatif dönemde 4 hastada (%33,4) herhangi bir komplikasyon olmazken, 6 hastada (%50) geçici fasiyal parezi (GFP), 1 hastada (%8,3) tükürük fistülü (TF), 1 hastada (%8,3) hem GFP hem de TF gelişti. YP yapılan 12 hastanın postoperatif patoloji sonucu 9'unda (%75) plemorfik adenoma, 2'sinde (%16,7) Whartin tümörü, 1'inde (%0,8) asinik hücreli karsinom olarak geldi. Hastalar ortalama 18,75 ay takip edildi ve takiplerde nüks saptanmadı. Sonuç: Parotis bezi tümörlerinde preoperatif yapılan İİAB, selim-habis ayırımında etkili yöntem olmakla beraber yanılma payının da olabileceği görülmektedir. Benign parotis tümörlerinde yüzeyel parotidektomi yeterli bir cerrahi yöntemdir. İntraoperatif fasial sinir monitorizasyonu kullanmanın cerraha ameliyat sırasında güven hissi verdiği ve medikolegal yönden gerekli olduğu kanaatindeyiz.
Abstract
Objective: The aim of our study was to review 12 patients retrospectively,in whom we performed superficial parotidectomy for parotid benign tumor including their preoperative evaluation, surgical procedures performed, postoperative pathologic results, follow-up period and complications.Material and Method: Data of 12 patients who underwent superficial parotidectomy between November 2013 and October 2016 obtained from patient files and electronic records in the hospital's archives were reviewed, retrospectively. The age, gender, laterality and size of the tumor, smoking, preoperative fine needle aspiration biopsy (FNAB) results, type of incision,,facial nerve monitoring (FSM) status, duration of drainage, postoperative complications, and the operation technique was recorded.Results: Seven female (58.3%), and 5 (41.7 %) male patients underwent superficial parotidectomy (SP). The mean age of the cases was 42.2 years. Eight patients (66.6%) had pleomorphic adenoma or mixed epithelial cell, 2 (16.7%) Wharthin tumor and 2 (16.7%) patients could not be diagnosed based on FNAB results. During postoperative period any complication was not detected in 4 patients (33.4%), while transient facial palsy (n=6; 50%), salivary fistula and transient facial palsy (n=1; 8.3%) developed in respective number of patients. During postoperative period, pleomorphic adenoma was reported in 9 (75%), Wharthin tumor in 2 (16.7%), acinic cell carcinoma in 1 (0.8%) out of 12 patients who underwent superficial parotidectomy. Patients were followed up for a mean of 18.75 months and no recurrence was observed during follow-up period.Conclusion: Preoperative FNAB performed in cases with parotid gland tumors seems to be an effective method in distinguishing between malign and benign tumors with a possible margin of error. Superficial parotidectomy is an adequate surgical procedure for benign parotid tumors. We believe that using intraoperative FSM gives the surgeon a feeling of confidence during surgery and it is necessary in the medicolegal conditions.
