Özet

ERAS “Enhanced Recovery After Surgery”nin kısaltılmışhali olup, ERAS programları fonksiyonel iyileşmeyi hızlandırmave postoperatif sonuçları düzeltme amacı ile kanıtadayalı girişimlerin birlikte kullanıldığı perioperatif protokollerdir.Bu programlar multidisipliner ve multimodalgirişimleri içermektedir. Amaç hastaların cerrahi strestepkilerini azaltmak, postoperatif morbidite ve mortalite ilehastanede kalış süresini kısaltmak ve hastaların cerrahiyeilişkin algılamalarını olumlu yönde etkilemektir. Obstetrikteve perinatolojide ERAS protokolü deyince, sezaryen şu aniçin ilk ve tek akla gelen cerrahi girişimdir. ERAS derneğitarafından planlı veya plansız sezaryenden 30-60 dk. öncesindenbaşlayıp, annenin taburcu olmasına kadar geçensüre için (preoperatif, intraoperatif, postoperatif) sezaryendoğumda ERAS konsensus önerileri ile bir ERAS rehberioluşturulmuştur. ERAS komponentleri; ön bilgilendirmeninsağlanmasını, iyi perioperatif beslenmeyi ve hidrasyonu,minimal invazif cerrahi teknikleri, normotermiyi sağlamaçabalarını, postoperatif bulantı-kusmanın önlenmesini,idrar sondasının erken çıkarılmasını, postoperatif ağrınınetkin bir şekilde tedavisini, hızlı mobilizasyonu ve multidisipinerbir ekip tarafından planlanan ve yönetilen koordinebir perioperatif bakımı kapsamaktadır.

Abstract

ERAS is an acronym for ‘Enhanced Recovery After Surgery’ is an evidence based program and consists of perioperative practice protocols to speed up the functional healing and postoperative outcomes. These programs include multidisciplinary and multimodal interventions. The aim is to reduce patients’ stress responses to surgery, postoperative morbidity and mortality, duration of hospitalization and to affect patients’ perceptions of surgery in a positive way. When the ERAS protocol is mentioned in obstetric and perinatology, C-section is the first and the only conceivable surgical intervention. Starting from 30-60 minutes before an elective or an emergency C-section and until the patient’s discharge (including preoperative, intraoperative and postoperative care), ERAS Society has published clinical practice guidelines based on ERAS expert consensus. ERAS protocols include preoperative information and counseling, early enteral feeding, postoperative fluid management, minimal invasive surgical techniques, maintenance of normothermia, prevention of postoperative nausea and vomiting, early removal of urinary catheter, efficient management of postoperative pain, early mobilization and covers a coordinated perioperative care that is planned and managed by a multidisciplinary team.
