Comparison of BACTEC TB system and conventional methods for recovery of mycobacterium tuberculosis from clinical specimens

Klinik örneklerden Mvcobacterium tuberculosis'i en erken tanıtacak yöntemi ve bu yöntemin duyarlığını saptamak için, 2254 klinik örnek direkt veya dekontaminasyondan sonra BACTEC 12B ve Lowenstein-Jensen (LJ) besiyerine ekildi. Tüm örnekler asidorezistan boyama yöntemi ile boyandı. Konvansiyonel besiyeri 8 hafta inkübe edilerek haftada bir, BACTEC 12B besiyerleri büyüme indeksin değerini belirlemek için haftada iki kez incelendi. BACTEC TB sistem ve LJ besiyerinin her ikisinde üreyen bakteriler incelendiğinde 102 M. tuberculosis identifiye edildi. M. tuberculosis'in zolasyonu için yanızca tek bir yöntem yeterli olmadı. Yöntemlerin duyarlığı sırasıyla; BACTEC TB sistem için % 76.5; LJ kültür sistem için % 49.0; asidorezistan boyama yöntemi için % 43.3 olarak bulundu. M. tuberculosis'in izolasyonu için gerekli ortalama zaman; BACTEC TB sistem ile 11.6 gün, LJ kültür sistem ile 21.7 gündü. BACTEC TB sistemde kontaminasyon (% 80), LJ kültür sisteminden (% 4.3) daha sık bulundu. BACTEC TB sistem, komtaminasyon oranın yüksek olması dışında, daha duyarlı, daha pratik ve izolasyon için daha az zamana gereksinim gösteren bir sistem olarak bulundu.
Abstract
A total of 2254 clinical specimens was inoculated directly or after decontamination to BACTEC 12B vial and Lowenstein-Jensen (LJ) medium to' provide the earlier detection of Mycobacterium tuberculosis and maximum test sensitivity. All of the specimens were stained with acid-fast staining procedure. Conventional media were incubated and examined weekly for 8 weeks. BACTEC 12B vials were examined twice a week for providing the value of growth index. A hundred-two M. tuberculosis strains were identified in BACTEC TB system and LJ medium. A single method did not suffice for the isolation of the bacterium. The sensitivities of methods were 76.5 %, 49.0 %, 43.3 % with BACTEC TB, LJ culturing system, and acid-fast staining methods, respectively. The average of required period for isolation of M. tuberculosis was 11.6 days with BACTEC TB system and 21.7 days with LJ medium. The contamination in BACTEC TB system (8.0 %) was more frequent than in LJ medium (4.3 %). BACTEC TB system was found more sensitive, practical, and required short time, exluding contamination rate.
