Effect of intrathecal hyaluronidase in the management of tuberculous cranial arachnoiditis:An experimental study

Tüberküloz meninjitle oluşturulan kraniyal araknoiditin önlenmesinde intratekal hyaluronidazın etkisi köpek modelinde incelendi. Onsekiz erişkin köpek; kontrol, erken ve geç tedavi olmak üzere üç deney grubuna ayrıldı. Meninjit oluşturmak için Mvcobacterium tuberculosis süspansiyonu sisterna magna içerisine enjekte edildi. İki mi serum fizyolojik solüsyonunda 1000 İU hyaluronidaz intratekal yolla 15 gün aralıklarla üç kez verildi. Tedaviye erken tedavi grubunda inokülasyondan 15 gün sonra, geç tedavi grubunda 90 gün sonra başlandı. Tüm hayvanlar deneyin 6. ayında öldürüldü.Histolojik incelemede; kontrol ve erken tedavi grubundaki hayvanların çoğunda pia-araknoid ve subdural mesafede şiddetli [inflamatuvar reaksiyon ve orta veya şiddetli fibrosis varken, inokülasyondan 90 gün sonra hyaluronidaz verilmesinin pia-araknoidal inflamasyon ve fibrosisi Önlediği gözlendi. Sonuçlar; inflamatuvar proçesin geç seyrinde beyin-omurilik sıvısındaki hyaluronidazın infeksiyonun oluşturduğu araknoiditi önlediğini göstermiş olup intratekal olarak verilen bu ilacın intrakranial araknoidit ve bunun komplikasyonlannın tedavisinde etkili olabileceği düşünülmektedir.
Abstract
The efficacy of intrathecal hyaluronidase in preventing tuberculous meningitis-induced cranial arachnoiditis was studied in a canine model. Each of 18 adult mongrel dogs was assigned to one of three experimental groups; control, early and late treatment. Suspension of Mvcobacterium tuberculosis was injected into cisterna magna to induced meningitis. Hyaluronidase was administered intrathecally at three separate times, the injections were administered with intervals of 15 days in doses 1000 IU in 2 ml 0.9 % saline. Treatment was given on 15 days after the inoculation in the early treatment group and on 90 days in the late treatment group. All animals were sacrificed in the 6th month of the experiment. On histologic examination, most of the animals in the control and early treatment groups had severe inflammatory reaction in pia-arachnoid and subdural spaces beside moderate to severe fibrosis. In contrast, hyaluronidase given on the 90 days after inoculation prevented the pia-arachnoidal inflammatory reactions and fibrosis. The results showed that the presence of hyaluronidase in the cerebrospinal fluid would reverse the infection-induced arachnoiditis if it is employed at the late course of the inflammatory process and suggested that this drug administered intrathecally would be an effective treatment of intracranial arachnoiditis and its complications.
