Comparison of three culture methods for detection of vaginal group B streptococcal colonization

Erken neonatal sepsis ve meninjite neden olan grup B streptokokun vertikai geçişi, intrapartum antibiyotik kullanılması ile engellenebilir. Bundan dolayı, infekte bebek doğurma riskini azaltmak için alınan antibiyotiği sınırlamak, grup B streptokok ile kolonize gebeyi belirlemek için doğru yöntem geliştirilmelidir. Koyun kanlı agar (BA), 1.25 fig/ml trimetoprim + 23.75 sulfametoksazol içeren selektif koyun kanlı agar (SBA) ve Lim buyyonu (LB) (% 1 yeast extract, 10 ng/ml kolistin ve 15 Hg/rhl nalidiksik asit içeren Todd-Hewitt broth) besiyerinden oluşan üç kültür yöntemi, vaginal yakınması göz önüne alınmayan 577 kadından alınan vaginal örnek kullanılarak değerlendirildi. Vaginal örneklerin 16'sında (% 2.8) grup B streptokok izole, edildi (en az bir kültür yöntemi ile). BA, SBA ve LB yönteminin duyarlılığı sırasıyla % 31.3, % 31.3 and % 100 olarak bulundu. Grup B streptokok ile aşırı kolonizasyonlu kadınlarda BA, SBA ve LB yönteminin duyarlılığı sırasıyla % 36.4, % 27.3 and %İ00 olarak belirlendi. BA and SBA yönteminin 48 saat inkübasyonla duyarlılığı 24 saate göre değişmedi. Bu çalışma, LB'nun klinik örneklerden grup B streptokoku belirlemek için gold standart olarak kullanılabileceğini 9östermektedir.
Abstract
Vertical transmission of Group B streptococci, the most frequent cause of early neonatal sepsis and meningitis, can be interrupted by intrapartum antibiotics. However, accurate methods for detecting colonized women with Group B streptococci should be developed to limit the administration of antibiotics to those who are at risk of delivering an infected infant. The three culture methods using 5 % sheep blood agar (BA), selective sheep blood agar (SBA) containing 1.25 ixglml trimethoprim + 23.75 fig/ml sulfamethoxazole, and Lim broth (LB) (Todd-Hewitt broth containing 1 % yeast extract, 10 ng/ml colistin, and 15 ng/ml nalidixin acid) for vaginal specimens from 577 nonselective women were evaluated. A total of 16 swabs (2.8 %) were positive (by at least one method) for Group B streptococci. BA, SBA and LB method had a sensivity of31.3 %, 31.3 % and 100 %, respectively. In women with heavy colonization, BA, SBA and LB method had sensitivities of 36.3 %, 27.3 % and 100 %, respectively. The sensivity of BA and SBA plates in 24 h incubation was the same in 48 h. This study suggests that LB should be used as a gold standard for detecting Group B streptococci from suspected clinical specimens.
