Özet

Mikobakterilerin mikroskobik tanısında Ziehl-Neelsen (Z-N) ve Rodamin-Auramin (R-A) boyama yöntemlerinin, kültür sonuçlan standart alınarak karşılaştırılması amaçlanmıştır. Çeşitli kliniklerden gönderilen 220 balgam örneğinin 45'inin kültüründe üreme olmuştur. Duyarlılık, özgüllük, pozitif prediktif ve negatif prediktif değerler Z-N boyama yöntemi için sırasıyla % 68.9, % 100.0, % 100.0, % 92.5; R-A boyama yöntemi için ise % 86.7, % 94.3, % 79.6, % 96.5 olarak bulunmuştur. Her iki boyama yöntemine ait sonuçlar birlikte değerlendirildiğinde, bu değerlerin sırasıyla % 83.7, % 100.0, % 100.0 ve % 96.5 olduğu görülmüştür. R-A boyama yönteminde % 20.4 oranında yalancı pozitiflik gözlenirken Z-N boyama yönteminde yalancı pozitiflik saptanmamıştır. İki boyama yöntemi arasındaki uyum % 82 olarak bulunmuştur. Duyarlılığı Z-N boyama yönteminden daha yüksek olan R-A boyama yönteminin yalancı pozitiflik oranı dikkate alındığında .özellikle pozitif olgularda Z-N boyama yöntemine de gereksinim duyulacağı ve sonuçta her iki yöntemin birlikte kullanılması gerektiği sonucuna varılmıştır.

Abstract

Two staining procedures, Ziehl-Neelsen (Z-N) and Rhodamine-Auramine (R-A), were compared for the microscopical diagnosis of mycobacteria by using the results of culture as standard. Mycobacterial growth was observed in 45 of 220 sputum samples from various clinics. The sensitivity, specificity, positive predictive value and negative predictive value for Z-N uand R-A staining methods were found to be 68.9 % and 86.7 %, 100.0 % and 94.3 %, 100.0 % and 79.6 %, 92.5 % and 96.5 %, respectively. When the results of both staining methods were combined and evaluated together, the sensitivity, specifity, positive predictive value and negative predictive value were calculated as 83.7 %, 100.0 %, 100.0 % and 96.5 %, respectively. Although false positive results was obtained by 20.4 % in R-A method, no false positive result was obtained in Z-N method. The consistency between two staining methods was found to be 82%. When the false positive rate of R-A staining method, which is more sensitive than Z-N method, was taken into consideration, Z-N staining method was needed particulary in R-A positive specimens. It is concluded that both staining methods should be used together.
