Çocuk hastalarda A grubu beta-hemolitik streptokokların antibiyotik toleransı ( Ön çalışma )

Özet
Çocuk polikliniğine başvuran 123 akut tonsillit ve farinjitli çocuktan alınan boğaz kültürlerinden idantifiye edilen 33 A grubu beta-hemolitik streptokok susunda antibiyotik toleransı mikrodilüsyon yöntemi ile incelemeye alınmıştır. In vitro olarak penisilin, eritromisin ve azitromisin'e tolerans saptanmamıştır. Oniki olguya 10 gün süre ile fenoksimetilpenisilin (50 m/kg) sağaltımı uygulanmış ve 10 gün sonra sekiz olguda kültürde üreme olmamıştır. Yedi olguya eritromisin (50 mg/kg, 10 gün) verilmiş, 10 gün sonra kontrola gelen üç olguda kültürde üreme saptanmamıştır. Azitromisin (20 mg/kg, 5 gün) verilen altı olgudan üçünde kontrol kültüründe üreme olmamıştır. Sonuç olarak, kültür incelemesi sonucu 33 olguda in vitro olarak azitromisin duyarlılığı, 21 olguda penisilin ve eritromisin duyarlılığı incelenmiş ve bu antibiyotiklere tolerans gözlenmemiştir. Bu ön çalışmada Manisa bölgesinde streptokok suşlarında antibiyotik toleransı olmadığı izlenimi alınmıştır.
Abstract
Thirthy-three group A beta-hemolytic Streptococcus strains obtained from the throat culture of 123 children with acute tonsillitis and pharyngitis referring to the Pédiatrie Outpatient Clinic was studied for antibiotic tolerance using microdilution method. In vitro tolerance to penicillin, erythromycin and azithromycin was not detected. Ten-day phenoxymethylpenicillin (50 mg/kmg) therapy was instituted to 12 cases and no culture growth was seen after 10 days in eight cases. Erythromycin (50 mg/kg, 10 days) was given to seven cases and no growth in the control culture was seen in three patients who came for control. There was no growth in the control culture of the three of the six patients to whom azithromycin (20 mg/kg, 5 days) was instituted. In conclusion, in vitro azithromycin susceptibility in 33 cases, penicillin and erythromycin susceptibility in 21 cases were studied and tolerance was not seen to any of these antibiotics. With this preliminary study, the impression was that no antibiotic tolerance of streptococci has yet developed in the Manisa Region.
