İn vitro amfoterisin B duyarlılık testlerinde '' Antibiotic medium 3 '' besiyerinin kullanılması

Klinik maya kökenlerinin amföterisin B'ye in_vitro duyarlılığının belirlenmesindeki sorunlar, RPM11640 besiyerinin kul-lanıldığı NCCLS yöntemi ile dirençli suşların duyarlı suşlardan ayırdedilememesi ve henüz kesin olarak belirlenmiş bir minimum inhibitor konsantrasyon (MIC) direnç sınır değerinin bulunmayışıdır. Bu çalışma, İnfeksiyon etkeni olduğu gösterilmiş klinik Candida izolatlarının amfoterisin B'ye duyarlılığının belirlenmesinde "Antibiotic Medium 3" (ABM3) besiyerinin değerini belirlemek ve olası dirençli suşları saptamak amacıyla planlanmıştır. Çeşitli klinik örneklerden izole edilen 81 Candida izolatı (43 C. albicans. 15 C. krusei. 12 C. kefyr. altı C. glabrata. dört C. tropicalis ve bir & parapsilosis) alınmış, amfoterisin B duyarlılığı NCCLS mikrodilüsyon ve E testi ile araştırılmıştır. Her iki yöntem, FtPMI 1640 ve ABM3 besiyeri kullanılarak test edilmiş, yöntemler ve besiyerleri arasındaki uyum değerlendirilmitir. MIC değerlerinin uygulanan duyarlılık testlerinde $geq$ 0.03-2 $mu$ g/mL arasında değiştiği, suşların çoğunda $geq$ 1 $mu$ g/ml olduğu görülmüştür. ABM3 besiyeri ile MIC değerinde belirgin yükselme saptanan izolat olmamıştır, iki besiyeri sonuçları arasındaki uyumluluk mikrodilüsyon ve E testi için sırasıyla % 93.8 ve % 92.6 olarak saptanmıştır. Bu sonuçlar, test edilen suşlar arasında amfoterisin B'ye olası dirençli izolat bulunmadığını ve ABM3 besiyerinin duyarlı suşlar için RPM11640 besiyerine benzer sonuçlar verdiğini göstermektedir.
Abstract
The inability of RPMI 1640 medium in discrimination of susceptible and resistant isolates and the lack of a definite minimum inhibitory concentration (MIC) breakpoint value constitute the current problems for antifungal susceptibility testing of amphotericin B. The present study was designed to establish the value of antibiotic medium 3 (ABM3) in amphotericin B susceptibility testing and to detect putatively resistant clinical Candida isolates. Eighty-one Candida strains (43 C. albicans. 15 C. krusei. 12 C. kefyr. six C. glabrata. four C. tropicalis and one C. parapsilosis) isolated from various clinical samples were included in the study. Amphotericin B susceptibility tests were performed by NCCLS microdilution method and E test, using RPMI 1640 and ABM3 as the test medium. The percent agreement between the two media was evaluated to determine the distribution of MIC values in susceptible strains and the presence of resistant strains. MIC values varied between $geq$ 0.03 and 2 $mu$ g/mL, being $geq$ 1 $mu$ g/mL for most of the isolates. None of the strains showed a prominent increase in MIC value when tested with ABM3. Percent agreement between the two media was found to be 93.8 and 92.6 % for microdilution and E test, respectively. The results indicate that there is no putatively amphotericin B resistant isolate among those included in the study and ABM3 gives comparable results to RPM11640 in'testing susceptible isolates.
