The evaluation of the smear and culture results of mycobacteriological specimens in a seven-year study period: A retrospective study

Özet

Yedi yıllık bir süre içerisinde mikobakteriyoloji laboratuvarına gönderilen 27519 klinik örneğin smear ve kültür sonuçları değerlendirilmiştir. Respiratuvar kanala ait örneklerin oranı % 77.1 olarak bulunmuştur. 1701 (% 6.2) örnekte asido-rezistan basil (AFıB) üremiş, ancak bunlar tür düzeyine kadar identifiye edilmemişlerdir. Kültür-pozitif olan 1221 (% 71.8) örneğin aynı zamanda smear sonuçlan da pozitif bulunmuştur. ARB'lerin % 90'ı respiratuvar örneklerden izole edilmiştir. Pozitif smear'lerin, daha çok kültür-pozitif respiratuvar örneklerle birliktelik gösterdikleri saptanmıştır. Smear sonucu pozitif olan 313 (% 1.1) örneğin kültürlerinde üreme saptanmamıştır. Smear duyarlılığı ve özgüllüğü sırasıyla % 71.8 ve % 98.8 olarak bulunmuştur. Kültür-pozitif örneklerin yıllık oranları kıyaslandığında, yedi yıllık süreye ait yıllık oranlardaki farklılıkların istatistiksel olarak anlamlı olmadığı bulunmuştur (p > 0.05). Bu bulgu, kültüre alman örneklerdeki yıllık ARB izolasyon oranlarının (ya da prevalansının), bu yedi yıllık süre içerisinde anlamlı bir şekilde değişmemiş olabileceğini düşündürmektedir.

Abstract

The smear and culture results of 27519 clinical specimens, which were submitted to the mycobacteriology laboratory in a seven-year period, were evaluated. The 77.1 % of the all submitted specimens was from respiratory tract. Acid-fast bacilli (AFB) grew from 1701 (6.2 %) of the specimens and they were not identified to species level. The 1221 (71.8 %) culture-positive specimens had also positive acid-fast smears. The 90 % of recovered AFB was from respiratory specimens. The positive smears were more frequently associated with culture-positive respiratory specimens than with other specimen types. The 313 (1.1 %) positive AFB smears were associated with negative cultures. The overall sensitivity and specificity of the smear was 71.8 % and 98.8 %, respectively. When the annual rates of culture positive specimens were compared, the varying rates were not statistically significant (p > 0.05). This finding suggests that the annual laboratory yield (or prevalence) of AFB in cultured specimens might not have changed significantly during the seven-year study period.
